目的对1例B_w血型用血清学及基因检测的方法进行鉴定。方法使用单克隆抗-A、B、D、AB、H和反定试剂红细胞以及人源抗-B,用血清学的方法对血样进行血型定型;采用分子生物学PCR-RFLP法对ABO基因第6、7外显子进行测序分析。结果患者血样血型血清学鉴定时正定抗-B较弱,且正反定不符;吸收放散实验证明患者红细胞表面存在B抗原;患者标本基因测序显示为Bw/O,并在721C>T突变。结论血型血清学与分子生物学实验相结合确定患者血型为B_w(03)/O,建议输注O型洗涤红细胞和AB型血浆。
Objective To identify a case of B_w blood by serological and genetic testing methods. Methods Blood samples were determined by serological method using monoclonal anti-A, B, D, AB, H and anti-reagent erythrocytes as well as human anti-B. Blood samples were determined by PCR-RFLP 6,7 exons for sequencing analysis. Results The blood samples of patients with positive serotype B were weaker than those of positive ones, and positive and negative were inconsistent. Antigen B was detected on the surface of erythrocytes by absorption and desorption tests. The gene sequence of the samples was Bw / O and was mutated at 721 C> T. Conclusion Blood group serology and molecular biology experiments to determine the patient’s blood group B_w (03) / O, recommended infusion of O-type red blood cells and AB plasma.
