目的探讨18F-脱氧葡萄糖(FDG)PET延迟显像对肺良恶性病变的鉴别诊断价值。方法对36例肺癌患者和20例肺良性病变患者分别进行18F-FDG PET早期头部至盆腔显像和注射后3h胸部延迟显像,测定两次显像病灶的标准摄取值(SUV),并计算18F-FDG的储留指数(RI)。结果肺癌组延迟显像SUV明显高于肺良性病变组(P<0.01),肺癌组的RI也明显高于肺良性病变组(P<0.01);对于早期显像SUV>2.5的肺部结节,以RI>20%为阈值,延迟显像对肺良恶性病变鉴别诊断的灵敏度、特异性和准确性分别为90.0%、80.0%和86.7%。结论18F-FDG PET延迟显像有助于肺良恶性病变的鉴别诊断,结合应用SUV及RI两项指标可提高诊断准确率。
Objective To investigate the differential diagnosis of benign and malignant lung lesions by 18F-deoxyglucose (FDG) PET delayed imaging. Methods Thirty-six patients with lung cancer and 20 patients with benign lung disease underwent early chest to pelvic imaging of 18F-FDG PET and delayed thoracic imaging at 3 hours after injection, and the standard imaging values (SUVs) of two imaging lesions were determined. The storage index (RI) of 18F-FDG was calculated. Results The delayed imaging of lung cancer group was significantly higher than that of benign lung disease group (P <0.01), and the RI of lung cancer group was significantly higher than that of benign lung group (P <0.01). For early imaging of pulmonary nodules with SUV> 2.5 The sensitivity, specificity and accuracy of delayed imaging in the differential diagnosis of benign and malignant lung lesions were 90.0%, 80.0% and 86.7%, respectively, with RI> 20% as the threshold value. Conclusions 18F-FDG PET delayed imaging is helpful to the differential diagnosis of benign and malignant lung diseases. Combined with SUV and RI, the diagnostic accuracy can be improved.
