目的:比较局部封闭注射和口服糖皮质激素治疗肉芽肿性小叶性乳腺炎(GLM)的临床疗效。方法:选取2017年8月—2019年8月湖南师范大学附属第一医院(湖南省人民医院)、湖南省中医药大学附属第一医院、湖南省浏阳市人民医院、湖南省祁东县人民医院及湖南省凤凰县人民医院收治的120例GLM患者,均为女性,采用随机数字表法将其分为两组:封闭组和口服组,每组各60例。封闭组患者予以曲安奈德封闭疗法联合手术治疗,口服组患者予以口服甲泼尼龙联合手术治疗。比较两组患者的术前有效率、起效时间及不良反应情况等方面的差异。计量资料以均数±标准差(n Mean±n SD)表示,组间比较采用n t检验;计数资料组间比较采用n χ2检验。n 结果:封闭组患者的有效率(80.0%)高于口服组(35.0%),封闭组患者的平均起效时间[(5.1±2.8) d]早于口服组[(7.5±2.5) d],封闭组患者的不良反应发生率(6.7%)低于口服组(21.7%),上述差异均具有统计学意义(n P<0.05)。n 结论:局部封闭较口服糖皮质激素具有起效快、有效率高、不良反应少的优势。“,”Objective:To compare the clinical efficacy of intralesional injection and oral corticosteroids in the treatment of granulomatous lobular mastitis(GLM).Methods:A analysis was made on 120 GLM patients admitted to the First Affiliated Hospital of Hunan Normal University (Hunan Provincial People′s Hospital), First Affiliated Hospital of Hunan University of Chinese Medicine, the People′s Hospital of Liuyang, the People′s Hospital of Qidong and the People′s Hospital of Fenghuang from August 2017 to August 2019. They were all female. They were randomly divided into two groups: closed group (n n=60) and oral group (n n=60). The patients in the closed group were treated with triamcinolone acetonide combined with surgery, and the patients in the oral group were treated with oral methylprednisolone combined with surgery. The preoperative effective rate, effective time and adverse reactions of the two groups were compared. The measurement data were expressed by n Mean±n SD, t-test was used for inter-group comparison, and Chi-square test was used for inter-group comparison of counting data.n Results:The effective rate in the closed group (80.0%) was higher than that in the oral group (35.0%). The average effective time in the closed group [(5.1±2.8) d] was earlier than that in the oral group [(7.5 ±2.5) d]. The adverse reaction rate in the closed group (6.7%) was lower than that in the oral group (21.7%), and the difference was statistically significant (n P<0.05).n Conclusion:Compared with oral corticosteroids, intralesional injection has the advantages of faster effect, higher effective rate and less adverse reactions.
