目的观察评估电子支气管镜检查和肺泡灌洗术对儿童大叶性肺炎的诊治效果。方法回顾性分析本院明确诊断为大叶性肺炎的128例患儿的临床资料,收集患儿电子支气管镜检查及支气管肺泡灌洗术资料,并对比电子支气管镜治疗与非电子支气管镜治疗的临床疗效。结果灌洗组患儿总热程、症状好转时间、体征消失时间、CRP恢复时间及住院天数均低于对照组(P均<0.05),灌洗组患儿有效率100%;对照组有效率94.3%,两组疗效比较,具有明显统计学差异(χ2=10.395,P=0.006)。患儿BALF病原学检查FQMP-DNA检测阳性者占46.6%,细菌培养阳性结果占27.6%。结论早期进行电子支气管镜诊治,能够明确病原,有效缓解症状和体征,促进炎症吸收,缩短病程,达到诊断及治疗的双重收益。
Objective To evaluate the diagnostic value of electronic bronchoscopy and bronchoalveolar lavage on children with lobar pneumonia. Methods The clinical data of 128 children diagnosed as lobar pneumonia in our hospital were retrospectively analyzed. The data of electronic bronchoscopy and bronchoalveolar lavage were collected and compared with those of electronic bronchoscopy and non-electronic bronchoscopy Clinical efficacy. Results The overall course of fever, symptom improvement time, disappearance of signs, recovery time of CRP and days of hospitalization in the lavage group were all lower than those in the control group (all P <0.05). The effective rate of the lavage group was 100% 94.3%. There was significant difference between the two groups (χ2 = 10.395, P = 0.006). Children with BALF etiology FQMP-DNA test positive for 46.6%, bacterial culture positive results accounted for 27.6%. Conclusion Early diagnosis and treatment of electronic bronchoscopy can clear the pathogen, effectively relieve the symptoms and signs, promote the absorption of inflammation, shorten the course of the disease, and achieve the double benefit of diagnosis and treatment.
