目的:探讨颅内前颞部皮样囊肿伴皮肤窦道的诊断和治疗。方法:分析在我科治疗的2例颅内前颞部皮样囊肿伴皮肤窦道患者的病史,临床治疗经过,并复习相关文献资料进行讨论。结果:病例1,女,3岁,因右颅内脓肿而就诊。病例2,男,5岁,因右颞间隙感染就诊。2例病例右前颞部皮肤均有窦道,感染控制后行颅内外联合手术,见囊肿条索均穿过颞骨与硬脑膜粘连。病理证实为皮样囊肿。术后分别随访14个月和3个月,均无复发。结论:前颞部皮样囊肿伴皮肤窦道非常罕见,会引起脑膜炎和脑脓肿等严重并发症,治疗不当易复发。需行CT或MRI检查,且施行颅内外联合手术,才能取得满意的治疗效果。
Objective: To investigate the diagnosis and treatment of anterior intracranial temporal dermoid cyst with sinus skin. Methods: We analyzed the history of 2 cases of anterior intracranial temporal dermoid cyst with sinus skin pathology treated in our department, and discussed the relevant literature after reviewing the clinical data. Results: Case 1, female, 3 years old, due to right intracranial abscess and treatment. Case 2, male, 5 years old, because of right temporal space infection treatment. In 2 cases, there were sinus in the right anterior temporal skin. After infection control, intracranial and extracranial surgery was performed. All the cyst and cords were found to pass through the temporal bone and dura. Pathological confirmed as dermoid cysts. The patients were followed up for 14 months and 3 months respectively, with no recurrence. Conclusion: The anterior temporal dermoid cyst with the skin sinus is very rare, which can cause serious complications such as meningitis and brain abscess. CT or MRI examination required, and the implementation of intracranial and external surgery, in order to obtain satisfactory treatment.
