心肌缺血及再灌注过程中出现的严重室性心律失常是冠心病猝死的重要原因,近年来对其发生机理及防治进行了广泛的研究。本文从心肌电生理学角度,对近3—5年来国内外研究进展作一简要回顾。一、缺血性心律失常的心肌电生理学研究结扎冠状动脉引起的急性缺血性心律失常早期(10min 内)可能由折返激动引起,在15—20min 时很可能有异常起搏活动产生。这两者都是由于心肌缺血后代谢改变所致,如由于氧分压降低、糖代谢由有氧氧化转为无氧酵解、细胞内高能磷酸化合物含量降低、代谢产物积聚等,从而引起心肌细胞电
