目的:观察云火活肾法联合高效血液透析治疗尿毒症患者顽固性皮肤瘙痒的临床疗效。方法:随机选取2011年10月~2013年7月在茂名市中医院肾内科门诊就诊的患者70例,根据随机对照原则分为治疗组和对照组,35例对照组患者通过高效透析进行治疗,35例治疗组患者通过云火活肾法联合高效透析进行治疗,对比两组患者皮肤瘙痒改善程度和生存质量。结果:治疗后治疗组患者瘙痒程度VAS评分(2.23+0.50)显著低于对照组患者(3.75+1.24)(P<0.05);治疗后治疗组患者生理功能、生理职能、躯体疼痛、生活活力、社会功能、情感职能、精神健康以及总体健康等各项生活质量评分均较治疗前显著提高(P<0.05),且显著高于对照组患者(P<0.05);治疗组患者治疗后BUN、Scr、β2-MG以及PTH等各项生化治疗均显著低于对照组患者(P<0.05)。结论:云火活肾法联合高效血液透析治疗尿毒症患者顽固性皮肤瘙痒临床疗效显著,值得进行推广。
Objective: To observe the clinical efficacy of Yunhuo Huoxue combined with hemodialysis in the treatment of refractory pruritus in uremic patients. Methods: Seventy patients randomly selected from Department of Nephrology, Renmin Hospital of Maoming, from October 2011 to July 2013 were randomly divided into treatment group and control group according to randomized control principle. 35 patients in control group were treated by high-efficiency dialysis, Thirty-five patients in the treatment group were treated with Yunhuo Huoxue method combined with high-efficiency dialysis. The improvement of pruritus and quality of life in the two groups were compared. Results: After treatment, the VAS score of pruritus in treatment group (2.23 + 0.50) was significantly lower than that in control group (3.75 + 1.24) (P <0.05); after treatment, the physical function, physical function, physical pain, Social quality, emotional function, mental health and overall health were significantly improved (P <0.05), and were significantly higher than those in the control group (P <0.05). After treatment, BUN, Scr , Β2-MG and PTH biochemical therapy were significantly lower than the control group (P <0.05). Conclusion: The treatment of refractory skin pruritus in patients with uremia by Yunhuo Hu Shen Decoction combined with high efficiency hemodialysis is clinically effective and worthy of promotion.
