目的:探索体表心电图特征用于鉴别左右室流出道心律失常的临床应用价值。方法:选取我院于2014年6月至2015年6月间收治的60例左右室流出道心律失常患者作为临床研究对象,对其进行体表心电图检测,分别对比分析R/S振幅指数、R波时限指数、胸导联移行区指数、V2转换转换率以及V2S/V3R指数,用于评估患者体表心电图以此鉴别左右室流出道心律失常。结果:以V1R振幅指数≥30%为标准判断,OTVA起源于左室流出道患者21例,经消融确定的患者17例;被诊断为起源于右室流出道患者39例,经消融确定的患者36例;以V1导联时限指数50%为诊断标准,OTVA起源于左室流出道患者13例,经消融确定的患者11例,其余47例右室流出道患者经消融确定的共计43例;以胸导联移行区指数低于0分为诊断标准,25例被诊断为左室流出道,经消融确定的19例,35例右室流出道患者经消融确定31例;以V2S/V3R指数<1.5为诊断标准19例诊断为左室流出道患者,消融确定15例,41例右室流出道患者经消融确定36例。结论:在临床实践上,体表心电图特征鉴别左右室流出道心律失常具有操作简单、准确率高等特点,为此,在临床实践中值得推广应用。
Objective: To explore the clinical value of surface electrocardiogram (ECG) for identifying arrhythmia of left and right ventricular outflow tract. Methods: Sixty patients with left-right ventricular arrhythmias admitted to our hospital from June 2014 to June 2015 were selected as the clinical study subjects. The electrocardiogram (ECG) of the patients was measured. The R / S amplitude index, R Wave time limit index, chest lead transition zone index, V2 conversion conversion rate and V2S / V3R index, used to assess the patient’s surface ECG in order to identify the left and right ventricular arrhythmias. Results: According to the V1R amplitude index≥30%, OTVA originated from 21 cases of left ventricular outflow tract and 17 cases were confirmed by ablation; 39 cases of patients diagnosed as having right ventricular outflow tract were diagnosed by ablation 36 cases. According to the diagnostic criteria of 50% V1 lead time index, OTVA originated from 13 cases of left ventricular outflow tract, 11 cases were confirmed by ablation, and 47 cases of right ventricular outflow tract were determined by ablation. Thirty-one cases were diagnosed as left ventricular outflow tract by ablation, 31 cases were confirmed by ablation, 35 cases were diagnosed as ablation of right ventricular outflow tract, and V2S / V3R index <1.5 as the diagnostic criteria for the diagnosis of 19 cases of left ventricular outflow tract ablation, ablation identified 15 cases, 41 cases of right ventricular outflow tract ablation identified 36 cases. Conclusion: In clinical practice, the characteristics of body surface electrocardiogram to identify left ventricular arrhythmia of right and left ventricular outflow tract has the characteristics of simple operation and high accuracy. Therefore, it is worthy of popularization and application in clinical practice.
