目的为了解HBV慢性感染免疫耐受期患者肝内NK细胞及Kupffer细胞的变化特点,探讨肝内NK细胞及Kupffer细胞与HBV慢性感染免疫耐受的关系。方法采用免疫组织化学方法检测16例HBV感染免疫耐受期患者肝组织内NK细胞及Kupffer细胞在肝内分布情况,以及HBsAg、HBcAg在肝细胞内表达状况,并与6例正常肝组织、17例免疫活动期患者进行比较。结果免疫耐受期患者肝内浸润Kupffer细胞明显多于正常肝组织(P<0.01),但明显少于免疫活动期患者(P<0.01);免疫耐受期患者肝内NK细胞数与免疫活动期及正常肝组织比较均差异无显著性(P>0.05);免疫耐受期患者肝细胞内HBcAg阳性表达明显高于免疫活动期患者(P<0.01)。结论肝组织内Kupffer细胞在慢性乙型肝炎的肝组织炎症损伤及肝内HBV清除中起重要作用,而慢性HBV感染者肝组织内NK细胞的作用很有限。
Objective To investigate the changes of intrahepatic NK cells and Kupffer cells in patients with chronic hepatitis B virus immune tolerance and to explore the relationship between intrahepatic NK cells and Kupffer cells and immune tolerance of chronic HBV infection. Methods Immunohistochemistry was used to detect the intrahepatic distribution of NK cells and Kupffer cells in 16 patients with HBV-infected immune tolerance and the expression of HBsAg and HBcAg in hepatocytes. Sixteen normal liver tissues, 17 Cases of immune active patients were compared. Results The number of intrahepatic infiltrate Kupffer cells in immunocompromised patients was significantly higher than that in normal liver tissues (P <0.01), but significantly lower than that in immunocompetent patients (P <0.01). The number of NK cells in immunocompromised patients and immune activity (P> 0.05). The expression of HBcAg in hepatocytes in immunocompromised patients was significantly higher than that in immunocompetent patients (P <0.01). Conclusions Kupffer cells in liver tissue play an important role in inflammation of liver tissues and HBV clearance in chronic hepatitis B patients. However, the role of NK cells in liver tissues of patients with chronic HBV infection is limited.
