目的:探讨治疗前n 18F-脱氧葡萄糖(n 18F-FDG)正电子发射计算机断层扫描(PET-CT)在结外NK/T细胞淋巴瘤中的临床应用价值。n 方法:收集2006年8月至2017年12月于中国医学科学院肿瘤医院就诊的、经病理证实诊断为结外NK/T细胞淋巴瘤患者(81例)的临床病理资料,采用Mann-Whitney n U检验和Spearman相关分析评价最大标准摄取值(SUVn max)与预后的关系。n 结果:全组81例患者中,上呼吸消化道受累80例,上呼吸消化道外受累7例,受累部位包括腮腺(1例)、乳腺(1例)、脾脏(1例)、胰腺(1例)、皮肤及皮下软组织(1例)、肌肉(1例)、肺(2例)、骨骼(3例),淋巴结受累33例。所有病灶均伴放射性摄取增高,全组患者中位SUVn max为8.6。15例患者因PET-CT改变临床分期,其中12例患者调整了治疗方案。21例患者因PET-CT改变了放疗靶区。全组患者1、2年无进展生存率分别为88.7%和80.3%,1、2年生存率分别为97.2%和94.4%。有局部淋巴结侵犯患者的中位SUVn max(11.1)与无局部淋巴结侵犯患者(6.9)比较,差异有统计学意义(n P=0.007)。SUVn max与Ann Arbor分期、乳酸脱氢酶、Ki-67呈正相关(n r值分别为0.366、0.308和0.270, n P值分别为0.001、0.005和0.017),SUVn max与淋巴细胞计数、血红蛋白含量呈负相关(n r值分别为-0.324和-0.225,n P值分别为0.003和0.043)。n 结论:结外NK/T细胞淋巴瘤好发于上呼吸消化道为主的结外器官,具有明显嗜FDG性。与传统影像学检查比较,n 18F-FDG PET-CT可敏感而全面检出结外NK/T细胞淋巴瘤受累部位,协助临床准确分期、治疗方案制定。治疗前SUVn max与部分预后因子存在相关性,具有潜在预后评估价值。n “,”Objective:To investigate the clinical value of pretreatment n 18F-fluorodeoxy glucose positron emission tomography/computed tomography (n 18F-FDG PET-CT) in extranodal NK/T-cell lymphoma.n Methods:Eighty-one patients with pathologically confirmed extranodal NK/T-cell lymphoma and pretreatment with PET-CT scan in Cancer Hospital, Chinese Academy of Medical Sciences from August 2006 to December 2017 were enrolled in the study. The clinical, follow-up and imaging data were analyzed retrospectively. The relationship between maximum standard uptake value (SUVn max) and prognosis were evaluated by Mann-Whitney U test and Spearman rank correlation analysis.n Results:Among the 81 patients, 98.8% (80/81) were upper aerodigestive tract (UAT) involved. Lesions at extra-UAT sites were detected in 7 cases, involving parotid gland (n n=1), breast (n n=1), spleen (n n=1), pancreas (n n=1), skin and subcutaneous soft tissue (n n=1), muscle (n n=1), lung (n n=2) and bone (n n=3). Lymph node involvement were demonstrated in 33 cases. All of the lesions had increased uptake of PET, the median SUVn max was 8.6. PET-CT changed staging in 15 cases, and 12 cases were adjusted treatment methods. 21 cases were changed radiotherapy target because of PET-CT. The 1-, 2-year progression-free survival (PFS) rates were 88.7% and 80.3% while 1-, 2-year overall survival (OS) rates were 97.2% and 94.4% respectively. The median SUVn max of patients with local lymph nodes involvement was significantly higher than those without local lymph nodes involvement (n P=0.007). The SUVn max was positively associated with Ann Arbor stage (n r=0.366, n P=0.001), lactate dehydrogenase (n r=0.308, n P=0.005) and Ki-67 level (n r=0.270, n P=0.017). The SUVn max was inversely associated with lymphocyte count (n r=-0.324, n P=0.003) and hemoglobin content (n r=-0.225, n P=0.043).n Conclusions:Extranodal NK/T-cell lymphoma predominantly occurs in extra-nodal organs, mainly in the upper respiratory and gastrointestinal tracts, with marked FDG-addiction. Compared with conventional imaging, n 18F-FDG PET-CT is sensitive and comprehensive in detecting extra-nodal NK/T-cell lymphoma involvement, assisting in accurate clinical staging and treatment planning. Pretreatment SUVn max is potential for prognosis evaluation since it is correlated with prognostic factors.n
