目的探讨新诊断的2型糖尿病(T2DM)患者脂肪组织SIRT1 mRNA表达水平及其与体质指数(BMI)、腰臀比(WHR)、血糖、血浆胰岛素、胰岛素抵抗指数(HOMA-IR)关系。方法采用RT PCR方法检测了40例对照组和40例T2DM患者脂肪组织SIRT1 mRNA水平,并分析了SIRT1水平与BMI、WHR、血脂、HbA_1c、血糖、血浆胰岛素和HOMA IR等的关系。结果新诊断的T2DM患者SIRT1 mRNA水平显著低于对照组(1.49±0.47 υs 1.12±0.32,P<0.01);线性相关分析表明,SIRT1与FIns、HOMA-IR呈显著负相关(r=0.421,P<0.01和r=-0.511,P<0.01)。以SIRT1为因变量,年龄、WHR、BMI、TG、TC、LDL-C、HDL-C、HbA_1c、FPG、FIns和HOMA-IR为自变量,进行多元线性逐步回归分析,结果表明HOMA-IR是影响SIRT1的独立相关因素。Logistic回归分析表明控制性别、年龄、WHR、BMI、TC、TG、HDL-C、LDL-C后,发现SIRT1与T2DM发病呈负相关,OR<1。结论脂肪组织中SIRT1 mRNA水平的变化与IR和T2DM相关。
Objective To investigate the expression of SIRT1 mRNA in adipose tissue of newly diagnosed type 2 diabetes mellitus (T2DM) patients and its relationship with body mass index (BMI), WHR, blood glucose, plasma insulin and insulin resistance index (HOMA-IR). Methods SIRT1 mRNA was detected by RT PCR in 40 cases of control group and 40 cases of T2DM. The relationship between SIRT1 level and BMI, WHR, blood lipid, HbA 1c, blood glucose, plasma insulin and HOMA IR were analyzed. Results SIRT1 mRNA levels in newly diagnosed T2DM patients were significantly lower than those in control group (1.49 ± 0.47 υs 1.12 ± 0.32, P <0.01). Linear correlation analysis showed that SIRT1 was negatively correlated with FIns and HOMA-IR (r = 0.421, P <0.01 and r = -0.511, P <0.01). Multivariate linear stepwise regression analysis showed that HOMA-IR was positively correlated with SIRT1 as a dependent variable, age, WHR, BMI, TG, TC, LDL-C, HDL-C, HbA 1c, FPG, FIns and HOMA-IR Independently related factors affecting SIRT1. Logistic regression analysis showed that there was a negative correlation between the incidence of T2DM and the incidence of T2DM (OR <1) after controlling sex, age, WHR, BMI, TC, TG, HDL-C and LDL-C. Conclusion The changes of SIRT1 mRNA in adipose tissue are correlated with IR and T2DM.
