百草枯(paraquat,PQ)是一种非选择性、接触性除草剂,近些年广泛应用于农业生产中。但是PQ对哺乳类动物有着极高的毒性,中毒的死亡率极高,病死率国外报道为40%~55%。PQ对肺脏、肾脏、皮肤、肝脏等全身多器官均有不同程度的损伤,早期尤以肺脏受损最为严重,常致使急性肺损伤(acute lung injury, ALI)或急性呼吸窘迫综合征(acute respiratory distress syndrome, ARDS)。PQ具体的致病机制尚未完全清楚,中毒早期肺损伤显著病理学特点是肺组织内大量炎性细胞聚集。因此减轻早期急性肺组织弥漫性炎症的扩散,阻碍后期百草枯肺纤维化发展的进程至关重要。糖皮质激素(glucocorticoid,GC)具有强大的抗炎和细胞效应,能抑制多种原因造成的炎症反应,减少各种炎性因子的释放。因此能在PQ中毒早期肺损伤的临床治疗中发挥重要作用。但是,目前对应用GC治疗急性PQ中毒意见不一,主要争论焦点为GC的应用剂量、给药时间窗的选择和疗程时间的长短等。至今GC治疗PQ中毒肺损伤缺乏临床大型的多中心双盲随机对照临床试验,其具体剂量、疗程、副作用、疗效分析等尚需进一步探讨。“,”Paraquat(PQ) is a non-selective contact herbicide,widely used in the domestic agricultural production in recent years.However, the PQ has a high toxicity for mammals and high poisoning mortality, which is 40% ~ 55% according to foreign report. PQ will cause different levels of multiple injuries to lungs,kidneys,skin,liver and other organs of the body. the most serious damage takes place especially in the lungs at early stage, often causing acute lung injury or acute respiratory distress syndrome. PQ pathogenic mechanisms are not clear, As early lung injury is characterized in pathology by inflammatory cell accumulation in the lung tissues,it is of great important to alleviate the proliferation of early acute diffuse inflammation and to obstruct or diminish the developing process of pulmonary fibrosis. Glucocorticoids (GC) are widely used in clinical practice, with a strong anti-inflammatory function and cellular effects, which can inhibit the inflammation caused by a variety of reasons, and reduce the release of various inflammatory factors. Therefore it plays an important role in the early clinical treatment of lung injury caused by PQ poisoning. However, the application of GC treatment for acute PQ poisoning remains controversial of which the main focus is GC dose, the therapeutic window and period of treatment,etc.As large multi-center double-blind randomized controlled clinical trials of GC treatment of lung injury caused by PQ poisoning are rare so far, further exploration is still needed to analyze the specific dose,cause of treatment and side effects, as well as efficacy.
