目的通过对广西省级综合医院2011年出院患者费用分析,了解住院费用特征及其影响因素,为政府和卫生部门控制高级别医院医疗费用过快增长、推进新医改进程、解决群众“看病难、看病贵”问题提供数据支撑和决策依据。方法收集2011年广西省级综合医院住院病例153 234例,采用描述性方法分析住院患者费用特征,采用多重线性回归分析影响住院费用的因素。结果广西省级综合医院住院患者的重点人群是中老年人,重点疾病是慢性非传染性疾病。住院费用昂贵、药费比例过高、劳务性医疗服务项目的费用比例较低现象严重。影响住院费用的主要因素是:住院天数、手术、年龄、入院情况、性别、输血、转科、院内感染、治疗结果、婚姻状况、医疗付款方式。结论住院费用受患者社会特征、临床特征和医院管理因素等多方面因素影响,因此住院费用的控制必须采取综合措施,以控制可控因素为突破口,加强对其的科学管理,同时对非可控因素进行规范和转化。

Hospitalization costs are high, the proportion of medical expenses is too high, and the proportion of the cost of labor-related medical services is low. The main factors affecting the cost of hospitalization are: days of hospitalization, surgery, age, admission, sex, blood transfusion, transfer of hospital, nosocomial infection, treatment outcome, marital status, medical payment method. Conclusion Hospitalization costs are affected by many factors, such as social characteristics, clinical characteristics and hospital management factors. Therefore, we must take comprehensive measures to control the cost of hospitalization, controllable factors as a breakthrough point, and strengthen its scientific management, while non-controllable Factors regulate and transform.
