目的观察缺血性脑血管病颅内、外动脉狭窄患者踝臂指数(Ankle-brachial index,ABI)的变化,探讨踝臂指数与颅内、外动脉狭窄的相关性。方法连续收集缺血性脑血管病患者104例,所有病例均行经颅多普勒(TCD)、头颈部CT血管成像(CTA)或核磁血管成像(MRA)确定是否存在颅内、外动脉狭窄并分组。记录患者的一般情况及血液学指标,并测量身高、体重、上肢及下肢血压,计算体重指数(BMI)和ABI。结果①不同程度颅内动脉狭窄组间ABI值差异无统计学意义(P=0.31﹥0.05)。②ABI值在颅外动脉狭窄各组间比较,重度狭窄组和其他组间差异有统计学意义(P﹤0.05),ABI和重度动脉狭窄组呈负相关(r=-0.673,P﹤0.05)。③ABI值在不同病变支数各组间比较,3支及以上狭窄组和无狭窄组、1支、2支狭窄组组间差异有统计学意义(P﹤0.05),ABI和3支及以上动脉狭窄组呈负相关(r=-0.747,P﹤0.05)。结论 ABI和颅内动脉狭窄无明显相关,和重度颅外动脉狭窄呈负相关,和3支及以上动脉狭窄呈负相关。
Objective To observe the changes of Ankle-brachial index (ABI) in patients with ischemic cerebrovascular disease complicated with intracranial and extracranial arterial stenosis and to explore the relationship between ankle-brachial index and intracranial and extra-arterial stenosis. Methods Totally 104 patients with ischemic cerebrovascular disease were collected. Transcranial Doppler (TCD), head and neck CT angiography (CTA) or nuclear magnetic resonance imaging (MRA) were used to determine the presence of intracranial and extracranial arterial stenosis And group. The patient’s general condition and hematologic parameters were recorded and the height, weight, upper and lower limbs blood pressure were measured, body mass index (BMI) and ABI were calculated. Results ① There was no significant difference in ABI between different groups of intracranial arterial stenosis (P = 0.31> 0.05). There was a significant difference between the two groups (P <0.05). There was a negative correlation between ABI and severe arterial stenosis group (r = -0.673, P <0.05). (3) There were significant differences in ABI between groups with different lesion count (P <0.05). There were significant differences between ABI and three or more arteries in three or more stenosis groups and no stenosis group, one or two stenosis groups The stenosis group was negatively correlated (r = -0.747, P <0.05). Conclusion There is no significant correlation between ABI and intracranial arterial stenosis, which is negatively correlated with severe extracranial arterial stenosis and negatively correlated with arterial stenosis of 3 or more.
