[目的]观察胃癌细胞p27kip1、cyclinE的表达情况,并分析幽门螺杆菌(Helicobacter pylori,Hp)感染对二者表达的影响,以探讨Hp可能的致癌机制。[方法]用免疫组织化学SP法检测59例胃癌p27kip1、cyclinE表达情况,采用改良Giemsa快速尿素酶检验法检测Hp感染。[结果]p27kip1阳性率在正常胃黏膜为84.22%,显著高于胃癌的47.46%(P<0.05),胃癌中高中分化腺癌组为68.42%,显著高于低分化腺癌的37.5%和与胃黏液癌的31.25%(P<0.05),但后两者间无显著性差异(P>0.05)。cyclinE阳性率在正常胃粘膜为11.11%,显著低于胃癌的55.93%(P<0.05),胃癌中高中分化腺癌组为31.58%显著低于低分化腺癌的66.67%和胃黏液癌的68.75%(P<0.05),后两者间无显著性差异(P>0.05)。p27kip1及cyclinE的阳性率在Hp阳性和Hp阴性胃癌组间无显著差异性(P>0.05)。[结论]胃癌组织p27kip1表达下调、cyclinE表达上调,提示二者的表达失衡可能会促进胃癌的发生。但二者表达与Hp感染无明显相关关系。
[Objective] To observe the expression of p27kip1 and cyclinE in gastric cancer cells and analyze the effect of Helicobacter pylori (Hp) infection on the expression of Hp and p27kip1 in order to investigate the possible carcinogenic mechanism of Hp. [Method] The expressions of p27kip1 and cyclinE in 59 cases of gastric cancer were detected by immunohistochemical SP method. Hp infection was detected by modified Giemsa rapid urease test. [Results] The positive rate of p27kip1 was 84.22% in normal gastric mucosa, which was significantly higher than that in gastric cancer 47.46% (P <0.05). The positive rate of p27kip1 in high gastric cancer group was 68.42%, significantly higher than that in poorly differentiated adenocarcinoma Gastric mucinous carcinoma of 31.25% (P <0.05), but there was no significant difference between the latter two (P> 0.05). The positive rate of cyclinE in normal gastric mucosa was 11.11%, which was significantly lower than that in gastric cancer 55.93% (P <0.05). The percentage of high and middle differentiated adenocarcinoma in gastric cancer was 31.58%, which was significantly lower than 66.67% in poorly differentiated adenocarcinoma and 68.75 % (P <0.05), no significant difference between the latter two (P> 0.05). The positive rates of p27kip1 and cyclinE were not significantly different between Hp positive and Hp negative gastric cancer (P> 0.05). [Conclusion] The down-regulation of p27kip1 and up-regulation of cyclinE in gastric cancer suggest that the imbalance of p27kip1 expression may promote gastric cancer. However, there was no significant correlation between the two expression and Hp infection.
