输尿管光纤导管导引腹腔镜下输尿管上段切开取石术的应用研究

目的 探讨输尿管光纤导管导引腹腔镜下输尿管切开取石术的临床应用价值.方法 将120例输尿管上段结石患者分为三组:输尿管光纤导管组、输尿管导管组(对照组A)、无导管组(对照组B).观察输尿管光纤导管使用安全性,比较各组寻找输尿管及结石时间、手术时间、术中出血量、术中输尿管损伤率及术中其他组织器官损伤率.结果 输尿管光纤导管组及输尿管导管组(对照组A)两组使用安全性(感染率、输尿管穿孔)差异无显著性(P>0.05);输尿管光纤导管组寻找输尿管及结石时间和手术时间与输尿管导管组(对照组A)及无导管组(对照组B)比较差异有极显著性(P< 0.01);输尿管光纤导管组术中平均出血量与输尿管导管组(对照组A)比较差异无显著性(P> 0.05);与无导管组(对照组B)比较差异有极显著性(P< 0.01);输尿管光纤导管组术中输尿管损伤率与无导管组(对照组B)比较差异有极显著性(P< 0.01);三组术中其他组织器官(腹膜、肠道、腹膜后大血管等)损伤率差异无显著性(P> 0.05).结论 输尿管光纤导管能显著缩短腹腔镜下寻找输尿管及结石的时间,降低腹腔镜下输尿管相关手术的手术风险,促进腹腔镜下输尿管相关手术的发展与普及.
