目的:探讨喉癌切缘组织中Survivin和基质金属蛋白酶-2(MMP-2)的表达与喉癌局部复发的关系。方法:选择48例原发灶为喉鳞状细胞癌、手术切缘组织病理学肿瘤阴性的患者,对喉癌组织及切缘组织应用免疫组织化学法分别进行Survivin和MMP-2检测。结果:148例喉癌患者原发灶组织中Survivin和MMP-2的阳性率分别为70.83%(34/48)、66.67%(32/48);手术切缘组织中Survivin和MMP-2的阳性率分别为47.92%(23/48)、37.50%(18/48),2组之间差异有统计学意义(P<0.05)。2喉癌原发灶组织中Survivin(+)34例及MMP-2(+)32例的复发率分别为26.47%(9/34)和25.00%(8/32),高于阴性组的7.14%(1/14)和12.50%(2/16),差异无统计学意义。3喉癌切缘组织中Survivin(+)23例及MMP-2(+)18例的复发率分别为34.78%(8/23)和38.89%(7/18),高于阴性组的8.00%(2/25)和10.00%(3/30),差异有统计学意义(P<0.05)。相关分析提示,喉癌切缘组织中Survivin阳性和MMP-2阳性与喉鳞状细胞癌的局部复发密切相关。结论:喉癌手术切缘组织中Survivin阳性、MMP-2阳性者局部复发率高,二者在手术切缘的表达可作为预测喉癌局部复发的生物学标志。
Objective: To investigate the relationship between the expression of Survivin and matrix metalloproteinase-2 (MMP-2) in the marginal tissue of laryngeal cancer and the local recurrence of laryngeal carcinoma. Methods: Forty-eight patients with primary laryngeal squamous cell carcinoma (LSCC) and negative histopathologic margin of the surgical margin were selected for detection of Survivin and MMP-2 by immunohistochemistry. Results: The positive rates of Survivin and MMP-2 in primary laryngeal carcinoma were 70.83% (34/48) and 66.67% (32/48) in 148 laryngeal cancer patients respectively. The positive rates of Survivin and MMP-2 The rates were 47.92% (23/48) and 37.50% (18/48), respectively. There was significant difference between the two groups (P <0.05). The recurrence rates of Survivin (+) 34 and MMP-2 (+) 32 cases in primary laryngeal carcinoma were 26.47% (9/34) and 25.00% (8/32), respectively, which were higher than those in negative group % (1/14) and 12.50% (2/16), the difference was not statistically significant. The recurrence rates of Survivin (+) 23 cases and MMP-2 (+) 18 cases were 34.78% (8/23) and 38.89% (7/18), respectively, which were higher than those in the negative group (8.00% (2/25) and 10.00% (3/30) respectively, the difference was statistically significant (P <0.05). Correlation analysis showed that Survivin positive and MMP-2 positive were closely related to the local recurrence of laryngeal squamous cell carcinoma. CONCLUSIONS: Survivin is positive in the marginal tissues of laryngeal carcinoma, and the local recurrence rate is high in patients with positive MMP-2. The expression of the two in surgical margins can be used as a biomarker to predict the local recurrence of laryngeal carcinoma.
