食管癌切除术后发生吻合口-主动脉瘘是一种少见、难于救治的严重并发症。我院在1981~1985年间施行食管癌切除术373例,发生此并发症3例,发生率0.8%(3/373),其中2例来不及救治而死亡,1例进行第二次开胸手术,但于术后第八天再次大出血死亡。现报告如下。例1 李某,男性,50岁,食管中段癌。于1981年5月经左胸作食管癌切除,胃食管弓上吻合。术后第六天给少量流质饮食,患者出现左胸背隐痛及低热。胸透示左胸少量积液,第十天半流质饮食。第12天上午突然大量呕吐鲜血,数分钟内即死亡。
The occurrence of anastomosis-aortic fistula after resection of esophageal cancer is a rare and difficult-to-treat severe complication. Our hospital performed 373 cases of esophageal cancer resection between 1981 and 1985. Three cases of this complication occurred. The incidence was 0.8% (3/373). Two cases died prematurely and one patient had a second open thoracic operation. However, he died again on the eighth day after the operation. The report is as follows. Example 1 Lee, male, 50 years old, with esophageal cancer. In May 1981, the left chest was removed for esophageal cancer and the gastroesophageal anastomosis was achieved. A small amount of liquid diet was given on the sixth day after operation. The patient experienced left chest and back pain and low fever. The chest showed a small amount of fluid in the left chest and a semi-liquid diet on the tenth day. On the morning of the 12th day, she suddenly vomited a lot of blood and died within minutes.
