目的 比较慢性阻塞性肺疾病 (COPD)患者吸入 2 0 0 μg与 40 0 μg喘乐宁进行支气管舒张试验结果的一致性。方法 采用随机、交叉、自身对照方法对 40例COPD患者吸入 2 0 0 μg、40 0 μg喘乐宁进行支气管舒张试验 ,比较一秒钟用力呼气容积 (FEV1)绝对值的改变量、FEV1改善率、弥散 (DLCO)改善率、气道阻力 (Raw)改善率、呼气流速峰值 (PEF)改善率、舒张试验阳性率。结果 COPD患者吸入 2 0 0 μg、40 0 μg喘乐宁进行支气管舒张试验 ,两个剂量组FEV 1绝对值改变量、FEV1改善率、DLCO改善率、Raw改善率、PEF改善率、舒张试验阳性率比较差异均无显著性。急性发作期COPD患者FEV1绝对值改变量、FEV1改善率、舒张试验阳性率均高于稳定期COPD患者 (P <0 .0 0 1)。结论 吸入 2 0 0 μg喘乐宁既可保证舒张试验的效能 ,又可减少药物用量和减轻药物副作用 ,是一理想的选择。
Objective To compare the concordance of bronchodilator test results between inhaled 200 μg and 40 0 mg salbutamol in patients with chronic obstructive pulmonary disease (COPD). Methods Bronchodilator test was performed on 40 COPD patients with inhalation of 200 μg and 40 μg of levranib by a randomized, crossover and self-controlled method. The changes of forced expiratory volume (FEV1) absolute value per second were compared with those of FEV1 Rate of improvement of DLCO, improvement of airway resistance, improvement rate of peak expiratory flow rate (PEF) and positive rate of diastolic test. Results COPD patients received bronchial dilation test by inhalation of 200 μg and 40 μg salbutamol. The FEV 1 absolute change, FEV 1 improvement, DLCO improvement, Raw improvement, PEF improvement and diastolic test were positive in the two groups No significant difference in the rate of difference. The absolute changes of FEV1, the improvement rate of FEV1 and the positive rate of diastolic test in COPD patients with acute exacerbation were all higher than those in stable COPD patients (P <0.01). Conclusion Inhalation of 260 μg of salbutamol can not only ensure the efficacy of diastolic test, but also reduce the dosage of drugs and reduce side effects of drugs, which is an ideal choice.
