目的根据医院管理需要,以科室为基础,摸索按病种分组付费管理中需要重点监测病组的确定方法,为医疗质量管理提供方向。方法根据实际工作经验,通过比较分析,以管理成本效果最大化为原则进行筛选。结果按科室例数大于50例,同时该病组例数大于30例、差额率绝对值大于20%进行筛选,并结合科室、病组分别分析情况,确定8个科室的21个病组作为重点病组进行管理,其中神经内科的3个病组将作为今后监测、管理、分析的重中之重。结论医院在管理过程中应当正确认识费用所反映出的实质内容;建立有效的院内监测及质量管理运行机制;有关部门应建立反馈机构,适时调整费用标准。
