男患,45岁。因腹泻3天、四肢无力1天、呼吸困难2小时急诊入院。入院前3天出现腹泻,3～4次/日,稀便,无脓血,不发热。入院前1天突感双下肢无力,3小时后波及双上肢,呈迅速上行性加重,无尿便障碍。既往无类似发作史及服棉酚史。于1993年7月6日凌晨就诊。门诊查体:体温36.3℃,脉搏82次/分,血压18.6/10.6kPa。肥胖体型,意识清楚,查体合作,双肺呼吸音清晰,心率82次/分,律齐,无杂音,腹部无异常。神经系统:颈软,颅神经部无异常,四肢近端肌力
