[目的]探讨消化道肿瘤行内镜黏膜下剥离术(endoscopic submucosal dissection,ESD)治疗的安全性及手术操作体会。[方法]选取行ESD治疗的消化道黏膜层及黏膜下层肿瘤患者10例,对镜下手术方法及并发症进行总结。[结果]10例患者均完整剥离病灶,平均手术时间65(40~90)min,平均住院时间9(7~15)d。所有病例术中仅有少量渗血,经喷洒及电凝即可止血;1例穿孔,经保守治疗后痊愈。[结论]ESD治疗消化道黏膜层及黏膜肌层肿瘤是相对安全有效的,大部分患者的手术并发症可保守治愈。
[Objective] To investigate the safety and surgical operation experience of endoscopic submucosal dissection (ESD) in digestive tract tumors. [Methods] Ten patients with gastrointestinal mucosal and submucosal tumors treated with ESD were selected, and the microsurgical methods and complications were summarized. [Results] All 10 patients were completely detached and the average operation time was 65 (40-90) min. The average hospitalization time was 9 (7-15) days. In all cases, only a small amount of bleeding occurred during the operation. Hemorrhage could be stopped by spraying and electrocoagulation. One case was perforated and recovered after conservative treatment. [Conclusion] ESD treatment of gastrointestinal mucosa and musculo myometrial tumors is relatively safe and effective, and surgical complications in most patients can be conservatively cured.
