2型糖尿病患者甲状腺激素水平与血管并发症的相关性

目的探讨2型糖尿病患者甲状腺激素水平与血管并发症的关系。方法将521例2型糖尿病患者分为2组,甲状腺功能正常456例为正常组,合并亚临床甲状腺功能减退65例为甲减组。根据2组辅助检查及各项实验室检查指标,比较2组心血管疾病、脑血管疾病、糖尿病肾病、视网膜病变、外周血管病变发生率。结果 2组年龄、病程、空腹血糖水平比较差异无统计学意义(P>0.05),甲减组心血管疾病(72.3%)、脑血管疾病(15.4%)、糖尿病肾病(63.1%)、视网膜病变(43.1%)、外周动脉病变(47.8%)发生率高于正常组(61.8%、13.8%、43.0%、25.9%、31.8%),2组比较差异有统计学意义(P<0.05)。结论定期检查糖尿病患者甲状腺功能并及时治疗甲状腺功能减退,有助于降低糖尿病血管并发症的发生。 Objective To investigate the relationship between thyroid hormone levels and vascular complications in type 2 diabetic patients. Methods 521 patients with type 2 diabetes were divided into two groups, 456 patients with normal thyroid function were normal group, and 65 patients with subclinical hypothyroidism were hypothyroidism group. According to 2 sets of laboratory examinations and laboratory tests, the incidence of cardiovascular disease, cerebrovascular disease, diabetic nephropathy, retinopathy and peripheral vascular disease were compared. Results There was no significant difference in age, course of disease and fasting blood glucose between the two groups (P> 0.05), hypothyroidism (72.3%), cerebrovascular disease (15.4%), diabetic nephropathy (43.1%) and peripheral artery disease (47.8%) were significantly higher than those in the normal group (61.8%, 13.8%, 43.0%, 25.9% and 31.8%, respectively). There was significant difference between the two groups (P <0.05). Conclusion Regular examination of thyroid function in patients with diabetes and timely treatment of hypothyroidism, helps to reduce the incidence of diabetic vascular complications.
