目的探讨2型糖尿病患者血浆Meprin-α水平与合并冠心病的相关性。方法选择2009年10月至2013年2月在重庆西南医院老年科住院治疗2型糖尿病136例,同时将66例健康体检者(CON组)纳入研究。根据冠状动脉造影结果,将入选患者分为2型糖尿病组(T2DM组,85例)和2型糖尿病合并冠心病组(T2DM-CHD组,51例)。采集动脉粥样硬化相关危险因素并测定血清Meprin-α、踝臂指数及臂踝脉搏波传导速度。结果与CON组比较,T2DM和T2DM-CHD组Meprin-α和baPWV值均显著增高,而ABI值则显著降低(P<0.05),而与T2DM组比较,Meprin-α值显著增高,而ABI值则显著降低(P<0.05)。Spearman相关分析结果提示甘油三酯、LDL、糖化血红蛋白和Meprin-α与冠心病发病正相关,而ABI与冠心病发病呈明显负相关;以合并冠心病为因变量,结果显示Meprin-α对2型糖尿病合并冠心病有独立的预测价值。结论 2型糖尿病患者Meprin-α水平与合并冠心病呈正相关,而且可作为其独立预测指标。
Objective To investigate the correlation between plasma Meprin-α level and coronary heart disease in type 2 diabetic patients. Methods From October 2009 to February 2013, 136 cases of type 2 diabetes were treated in the department of geriatrics of Southwest Hospital of Chongqing. 66 healthy subjects (CON group) were included in the study. According to coronary angiography results, the patients were divided into type 2 diabetes group (T2DM group, 85 cases) and type 2 diabetic patients with coronary heart disease (T2DM-CHD group, 51 cases). Atherosclerosis-related risk factors were collected and serum Meprin-α, ankle-brachial index and brachial-ankle pulse wave velocity were measured. Results The levels of Meprin-α and baPWV in T2DM and T2DM-CHD group were significantly higher than those in CON group, while the ABI value was significantly lower (P <0.05), while Meprin-α was significantly higher in T2DM and T2DM-CHD group compared with CON group Was significantly lower (P <0.05). Spearman correlation analysis showed that triglyceride, LDL, HbA1c and Meprin-α were positively correlated with coronary heart disease, while ABI was negatively correlated with the incidence of coronary heart disease. Coronary heart disease was the dependent variable, and the results showed that Meprin- Diabetes with coronary heart disease have independent predictive value. Conclusion The level of Meprin-α in patients with type 2 diabetes mellitus is positively correlated with coronary heart disease, and can be used as an independent predictor.
