目的探讨实时剪切波弹性成像技术(SWE)对类风湿关节炎(RA)膝关节滑膜炎活动性程度的评价。方法选取RA膝关节滑膜炎患者36例,分别进行常规超声、能量多普勒(PDUS)和SWE检查,对比分析滑膜弹性模量值(EI)与血流分级的相关性。结果 36例共145个关节切面成功获取。根据血流分级为4组:0级27个切面,1级66个切面,2级43个切面,3级9个切面。EI分别为:(8.10±4.96)kPa、(12.83±5.63)kPa、(12.27±5.34)kPa和(15.77±5.30)kPa。0级与1、2、3级EI比较差异有统计学意义(P<0.05),1、2、3级组间比较差异无统计学意义。同一患者两侧及同侧各关节切面两两比较,差异均无统计学意义。结论结合PDUS血流分级,SWE技术可用于对RA患者膝关节滑膜炎活动性病情程度的评价。
Objective To evaluate the evaluation of knee joint synovitis activity in rheumatoid arthritis (RA) by real-time shear wave elastography (SWE). Methods Thirty - six patients with RA knee synovitis were selected for routine examinations of ultrasound, energy Doppler (PDUS) and SWE. The correlation between synovial elastic modulus (EI) and blood flow classification was analyzed. Results A total of 145 articular sections of 36 cases were successfully obtained. According to the blood flow classification for the 4 groups: 0 27 sections, a 66 sections, 2 43 sections, 3 9 sections. EI were (8.10 ± 4.96) kPa, (12.83 ± 5.63) kPa, (12.27 ± 5.34) kPa and (15.77 ± 5.30) kPa, respectively. Grade 0 and Grade 1, 2 and 3 EI were significantly different (P <0.05), while there was no significant difference between groups 1, 2 and 3. The two sides of the same patient and the ipsilateral joint section comparison, the differences were not statistically significant. Conclusion Combined with PDUS blood flow classification, SWE technique can be used to evaluate the degree of active disease of knee joint synovitis in patients with RA.
