目的:了解我国35岁及以上人群二尖瓣反流(MR)的检出情况及其影响因素,为高危人群的预防提供参考。方法:采用分层多阶段抽样的方法抽取35岁及以上的成年人作为调查对象,通过调查问卷及体格检查收集调查人群的一般情况、生活方式、疾病史、身高、体重等信息,并进行彩色多普勒超声心动图检查以及空腹血糖和血脂的测定。结果:纳入分析对象共28 814人,MR总体检出率为18.4%,其中男性和女性MR检出率均为18.4%。中重度MR检出率:35~50岁年龄组为0.3%,51~64岁年龄组为0.9%,≥65岁年龄组为2.2%。随年龄升高,MR检出率有升高的趋势。多因素Logistic回归分析发现年龄、收缩压、城乡、地区、左心房前后径、左心室舒张末期前后径、左心室射血分数、卒中病史、心房颤动病史、心力衰竭病史是MR的影响因素。结论:我国人群MR检出率较高,年龄、收缩压、城乡、地区、左心房前后径、左心室舒张末期前后径、左心室射血分数、卒中病史、心房颤动病史、心力衰竭病史是MR的主要影响因素,针对具有上述危险因素的人群应采取针对性的预防措施。
Objective: To understand the detection of mitral regurgitation (MR) and its influencing factors in people aged 35 and above in our country and provide reference for the prevention of high risk population. Methods: Stratified multi-stage sampling method was used to select adults aged 35 years and older as the survey subjects. The questionnaire and physical examination were used to collect general information, life style, disease history, height and weight of the surveyed population, Doppler echocardiography and fasting blood glucose and lipid profile. Results: A total of 28 814 subjects were included in the analysis. The overall detection rate of MR was 18.4%. The detection rate of MR in both men and women was 18.4%. The detection rate of moderate and severe MR was 0.3% in 35-50 years old group, 0.9% in 51-64 years old group and 2.2% in ≥65 years old group. With age, MR detection rate has increased. Multivariate Logistic regression analysis found that age, systolic blood pressure, urban and rural areas, regional left ventricular diastolic diameter, left ventricular end diastolic anteroposterior diameter, left ventricular ejection fraction, history of stroke, history of atrial fibrillation, history of heart failure are the influencing factors of MR. Conclusion: The detection rate of MR in Chinese population is higher than that in other countries. Age, systolic pressure, anteroposterior diameter of left atrium, anteroposterior diameter of left ventricular end-diastolic, left ventricular ejection fraction, history of stroke, history of atrial fibrillation and history of heart failure are MR Of the main influencing factors, targeted at the risk of the above groups of people should take targeted preventive measures.
