目的探讨Charlson指数(CCI)用于评估老年多发性骨髓瘤(MM)患者预后的价值。方法回顾性分析93例老年MM患者的临床资料,根据CCI评分分组,并对患者的临床特征及实验室数据进行单因素及多因素分析,采用Kaplan-Meier方法绘制生存曲线,Log-Rank检验比较不同CCI组别生存时间。结果单因素分析显示,年龄≥75岁,CCI评分≥5分、国际分期系统(ISS)分期Ⅲ期、Hb<100g/L、SCr≥176.8μmmol/L和乳酸脱氢酶(LDH)>245U/L均是影响患者预后的危险因素(P<0.05或P<0.01)。多因素分析显示,CCI评分≥5分和LDH>245U/L是影响患者预后的独立危险因素(P<0.01);CCI≥5分患者中位生存期23个月,短于CCI<5分患者的45个月(P<0.01)。结论 CCI评分用于评估老年MM患者预后具有临床应用价值。
Objective To investigate the value of Charlson Index (CCI) in assessing the prognosis of elderly patients with multiple myeloma (MM). Methods The clinical data of 93 elderly MM patients were retrospectively analyzed. According to the CCI score, the clinical characteristics and laboratory data of patients were analyzed by single factor and multivariate analysis. The survival curves were drawn by Kaplan-Meier method. Log-Rank test Different CCI group survival time. Results Univariate analysis showed that patients with age ≥75 years, CCI score ≥5, ISS stage Ⅲ, Hb <100g / L, SCr ≥176.8μmmol / L and lactate dehydrogenase (LDH)> 245U / L were risk factors affecting the prognosis of patients (P <0.05 or P <0.01). Multivariate analysis showed that the CCI score ≥5 points and LDH> 245U / L were independent risk factors (P <0.01). The median survival time of patients with CCI≥5 was 23 months, shorter than those with CCI <5 45 months (P <0.01). Conclusion The CCI score is of clinical value in assessing the prognosis of elderly patients with MM.
