目的应用螺旋CT肝脏动态扫描,比较不同注射速率对腹主动脉、肝脏强化程度的影响,以选择适合动脉期、门脉期的注射速率和最佳延迟时间。方法经B超检查后86例患者分为肝正常组和肝硬化组,对比剂量按1·5ml/kg计,注射速率(2ml/s、2·5ml/s、3ml/s、3·5ml/s),从肘静脉注射对比剂后,分别于15、20、25、30、65、70、75s在第一肝门水平扫描,测量腹主动脉、门静脉、肝实质的CT值,并做统计学分析。结果不同注射速率腹主动脉、门静脉、肝实质的强化程度不同,达到峰值时间也不同,以3ml/s组最优。结论延迟扫描时间应按不同注射速率而定,适合双期的扫描速率应为3ml/s,延迟时间动脉期为25s,门脉期为70s,但还应适当考虑患者自身情况。
Objective To evaluate the effects of different injection rates on the enhancement of abdominal aorta and liver in spiral CT hepatic dynamic scanning in order to select the suitable injection rate and optimal delay time for arterial phase and portal phase. Methods Eighty-six patients were divided into hepatic normal group and cirrhosis group by B-mode ultrasonography. The contrast dose was 1.5ml / kg, the injection rate (2ml / s, 2.5ml / s, 3ml / s). After contrast agent injection from the elbow vein, they were scanned at the level of the first hepatic portal at 15, 20, 25, 30, 65, 70 and 75 s respectively. CT values of the abdominal aorta, portal vein and liver parenchyma were measured and counted Analysis. Results Different injection rate of the abdominal aorta, portal vein, liver parenchyma enhancement, peak time is also different, 3ml / s group optimal. Conclusions Delayed scan time should be determined according to different injection rates. The appropriate scan rate should be 3ml / s for two-phase, 25s for delayed arterial phase, and 70s for portal phase. However, patients should be considered appropriately.
