目的:探讨腹腔镜下膀胱根治性切除Wang-Zebra(汪式-斑马,WZ)式乙状结肠原位新膀胱术尿流动力学指标的研究。方法:对36例患者行腹腔镜下根治性膀胱切WZ式乙状结肠原位新膀胱术,检测术后尿流率、剩余尿、新膀胱压力、膀胱容量,并记录患者术后12、24个月的控尿情况。结果:36例患者12、24个月的尿流动力学指标参数中最大尿流率分别为(15.50±1.50)、(15.15±1.25)ml/s,新膀胱排尿期最大膀胱压力分别为(6.15±0.37)、(7.21±0.34)kPa、新膀胱储尿期最大膀胱压力分别为(2.28±0.27),(2.93±0.24)kPa、新膀胱容量分别为(493.30±10.86)、(514.10±15.50)ml,剩余尿量为(61.27±3.39)、(57.53±3.10)ml。其中新膀胱容量、储尿期膀胱压力、排尿期膀胱压、剩余尿量比较差异有统计学意义(P<0.05)。最大尿流率两组比较差异无统计学意义(P>0.05)。结论:WZ式肠代新膀胱疗效确切,值得临床上推广运用。
Objective: To investigate the urodynamic characteristics of laparoscopic radical resection of Wang-Zebra (Wang-Zebra WZ) sigmoid colon in situ neobladder. Methods: Thirty-six patients underwent laparoscopic radical cystectomy with WZ sigmoid colon in situ neo-bladder surgery. The postoperative urinary flow rate, residual urine, new bladder pressure and bladder volume were recorded. The patients were followed up for 12 and 24 months Control of urine situation. Results: The maximum urinary flow rates in urodynamic parameters at 12 and 24 months in 36 patients were (15.50 ± 1.50) and (15.15 ± 1.25) ml / s, respectively. The maximum urinary urinary bladder bladder pressure in the 36 patients were 6.15 ± (2.37 ± 0.27) and (2.93 ± 0.24) kPa, respectively. The new bladder volume was (493.30 ± 10.86) and (514.10 ± 15.50) ml , Remaining urine volume (61.27 ± 3.39), (57.53 ± 3.10) ml. Among them, the capacity of new bladder, bladder pressure during storage, urinary bladder pressure during voiding period and residual urine volume were significantly different (P <0.05). The maximum urinary flow rate was no significant difference between the two groups (P> 0.05). Conclusion: WZ type of bladder on behalf of the new bladder effective, it is worth promoting the clinical application.
