目的探究GeneX-pertMtb/RIF检验、抗酸染色、BD960培养在结核病诊断和利福平耐药效能检测中的应用价值。方法选取2014年4月~2016年4月青海省地区确诊为结核病的患者352例,设为研究组,选取同期收治的非结核病患者46例作为对照组,分别采用抗酸染色涂片检验、GeneX-pertMtb/RIF检测和BD960培养进行准确性对比分析,并对比GeneX-pertMtb/RIF检测和BD960药敏结果。结果抗酸染色涂片检验阳性率36.6%,BD960检验阳性率54.5%,GeneX-pertMtb/RIF检验阳性率63.6%,GeneX-pertMtb/RIF检验>BD960检验>抗酸染色涂片检验,差异均有统计学意义(P<0.05);敏感性GeneX-pertMtb/RIF检验>BD960检验>抗酸染色涂片检验,但特异性对比,差异无统计学意义(P>0.05);BD960检验结果显示,352例患者中耐药率18.2%,GeneX-pertMtb/RIF检验结果显示耐药率20.2%,两组对比,差异无统计学意义(P>0.05)。结论GeneX-pertMtb/RIF检验具有简单快捷、准确性高,且能够同时进行结核菌排查及药敏检验,生物安全性要求低,不易造成交叉污染,相较于其他检验方案具有更好的优势,值得在临床上推广和应用。
Objective To investigate the value of GeneX-pertMtb / RIF test, acid-fast staining and BD960 culture in diagnosis of tuberculosis and detection of rifampicin resistance. Methods From April 2014 to April 2016, 352 patients diagnosed as TB in Qinghai Province were enrolled in this study. A total of 46 non-TB patients were enrolled in the study. Antacid staining and GeneX -pertMtb / RIF test and BD960 culture were compared and analyzed, and GeneX-pertMtb / RIF test and BD960 drug susceptibility results were compared. Results The positive rate of acid-fast staining was 36.6%, positive rate of BD960 was 54.5%, positive rate of GeneX-pertMtb / RIF was 63.6%, GeneX-pertMtb / RIF test> BD960 test> acid-fast staining test, (P <0.05); Sensitivity GeneX-pertMtb / RIF test> BD960 test> antacid stained smear test, but there was no significant difference in specificity (P> 0.05); BD960 test results showed that 352 The resistance rate was 18.2% in patients and 20.2% in GeneX-pertMtb / RIF test. There was no significant difference between the two groups (P> 0.05). Conclusion The GeneX-pertMtb / RIF test has the advantages of simple and quick, high accuracy, simultaneous detection of tuberculosis and susceptibility testing, low biosafety requirements, less cross-contamination and better advantages over other test protocols. Worthy of clinical promotion and application.
