目的 分析儿童腹型过敏性紫癜临床特点,减少误诊.方法 对本院1998年1月至2007年12月住院的46例儿童腹型过敏性紫癜的临床资料进行回顾性分析结果 临床表现以腹痛、消化道出血、腹泻分别为100%、47.8%和37.0%,腹痛症状表现严重而体征相对较轻,查体常无固定压痛点. 一过性关节肿痛发生率87.0%. 迟出现紫癜10例,特殊皮疹或紫癜认识不足7例,未发现紫癜5例共22例均造成误诊漏诊(48%). 误诊漏诊为急腹症11例占误诊漏诊50%.预后良好.结论 儿童腹型过敏性紫癜以腹痛为主要表现,常合并消化道出血,临床表现复杂,特别是消化道症状先于皮肤紫癜出现的误诊漏诊率高,易误诊为急腹症,应提高认识,尽早确诊,早期治疗.
