【目的】观察周期性与持续性营养输注方式对内科呼吸衰竭机械通气病人血糖及血清炎症因子的影响。【方法】采用交叉的自身对照方式测定60例内科呼吸衰竭机械通气患者周期性与持续性营养输注方式过程中血糖水平的变化及营养输注前后血清炎症因子水平变化,比较两种输注方式对机械通气患者血糖及血清炎症因子的影响。【结果】24h持续输注组总体血糖水平低于16h周期性输注组,分别为[(8.37±2.30)mmol/L,(8.89±2.77)mmol/L,P=0.000]。持续组各天血糖水平之间差异有统计学意义(P=0.011)而各点血糖水平之间的差别无统计学意义(P=0.096)。持续组与周期组血清TNF-α下降值水平为(13±5)、(8±3)pg/mL;血清IL-6下降值分别为(22±11)、(15±6)pg/mL,两组之间的差别有统计学意义(PTNF-α=0.000,PIL-6=0.017)。【结论】持续性营养输注方式下的血糖水平低于周期性营养输注,且全天血糖水平波动较小,利于病人血糖控制;持续性输注方式有助于改善血清炎症因子水平。
【Objective】 To observe the effects of periodic and continuous nutrition infusion on blood glucose and serum inflammatory factors in patients with mechanical respiratory failure. 【Methods】 The changes of blood glucose level and the level of serum inflammatory cytokines before and after nutrition infusion were measured in 60 patients with mechanical respiratory disease with mechanical ventilation during the period of periodic and continuous nutrition infusion by means of crossed self-control. Effect on Blood Glucose and Serum Inflammatory Factors in Patients with Mechanical Ventilation. 【Results】 The overall blood glucose level of 24h continuous infusion group was lower than that of 16h periodic infusion group [(8.37 ± 2.30) mmol / L, (8.89 ± 2.77) mmol / L, P = 0.000]. There was a significant difference in the level of blood glucose between the continuous group and the control group (P = 0.011). There was no significant difference in the blood glucose level between the two groups (P = 0.096). The levels of serum TNF-α in continuous group and periodic group were (13 ± 5) and (8 ± 3) pg / mL respectively, and the serum IL-6 levels were decreased by 22 ± 11 and 15 ± 6 pg / mL, respectively The differences between the two groups were statistically significant (PTNF-α = 0.000, PIL-6 = 0.017). 【Conclusion】 The blood glucose level under continuous nutrition infusion mode is lower than that of periodic nutrition infusion, and the whole blood glucose level fluctuation is small, which is beneficial to the patient’s blood glucose control. Continuous infusion mode can help to improve the level of serum inflammatory cytokines.
