目的 探讨磁共振成像 (MRI)、单光子断层扫描 (SPECT)及脑电图 (EEG) 3种不同方法对Tourette综合征的诊断价值。方法 收集以上 3种检查资料齐全的门诊 Tourette患者 2 8例 ,计算这 3种检查的阳性率和病变个数 ,并作统计学处理。结果 SPECT检查异常 2 0例 (71.4% ) ,多在颞叶 (8例 )、额叶 (7例 )、基底节 (6例 ) ;EEG明显异常 13例 (4 6 .7% ) ,表现为以 θ波为优势节律 ,伴有散在或阵发性 δ波活动 ,5例夹有尖波或尖慢波综合 ;无局灶性异常。 MRI异常 10例 (35 .7% ) ,共检出病灶 10个 ,其中基底节不对称 6例。结论 SPECT的敏感性最高 ,提示的病变最多 ;MRI的敏感性最低 ,所示的病灶最少。提示 SPECT是诊断 Tourette综合征的最佳检查指标。
Objective To investigate the diagnostic value of three different magnetic resonance imaging (MRI), single photon tomography (SPECT) and electroencephalogram (EEG) in the diagnosis of Tourette’s syndrome. Methods Twenty-eight outpatients with Tourette with the above three kinds of examinations were collected. The positive rates and the number of lesions in these three kinds of examinations were calculated and statistically analyzed. Results There were 20 cases (71.4%) of SPECT abnormalities in the temporal lobe (8 cases), frontal lobe (7 cases) and basal ganglia (6 cases). EEG abnormalities were found in 13 cases (46.7%), The theta wave as the dominant rhythm, accompanied by scattered or paroxysmal δ wave activity, 5 cases with spike or sharp wave synthesis; no focal abnormalities. MRI abnormalities in 10 cases (35.7%), a total of 10 lesions were detected, including 6 cases of basal ganglia asymmetry. Conclusions SPECT has the highest sensitivity and indicates the most lesions. MRI has the lowest sensitivity and minimal lesions. Tip SPECT is the best diagnostic indicator of Tourette’s syndrome.
