目的:探讨冠状动脉粥样硬化性心脏病(冠心病)患者踝肱指数(ABI)及肱踝脉搏波速度(baPWV)与冠状动脉造影Gensini评分的关系。方法:应用日本克林公司生产的全自动动脉硬化检测仪VP21000测量76例冠心病患者(冠心病组)和30名健康体检者(对照组)的baPWV、ABI;对冠心病患者进行冠状动脉造影,并通过Gensini评分对其冠状动脉病变进行评分。结果:与对照组比较,冠心病组ABI明显降低,baPWV明显升高(P<0.05);冠心病患者ABI与冠状动脉造影Gensini评分呈负相关(r=-0.686,P<0.001);baPWV与冠状动脉造影Gensini评分呈正相关(r=0.634,P<0.001);在对性别、年龄、高血压、吸烟、体质量指数、尿酸变量作偏相关分析后,ABI、baPWV与冠状动脉造影Gensini评分仍存在相关性,其相关系数分别为-0.703及0.665(P均<0.001)。结论:冠心病患者ABI降低及baPWV升高,ABI及baPWV分别与冠状动脉病变积分呈负相关及正相关,提示ABI及baPWV可作为评价冠状动脉病变程度的实用指标。
Objective: To investigate the relationship between ankle brachial index (ABI) and brachial-ankle pulse wave velocity (baPWV) and coronary angiography Gensini score in patients with coronary atherosclerotic heart disease (CHD). Methods: The baPWV and ABI of 76 patients with CHD and 30 healthy controls (control group) were measured by VP21000 manufactured by Kling Company of Japan. The coronary angiography , And their coronary artery lesions were scored by Gensini score. Results: Compared with control group, the ABI of coronary heart disease group was significantly lower and baPWV was significantly higher (P <0.05). The ABI was negatively correlated with coronary angiography Gensini score (r = -0.686, P <0.001) Coronary angiography was positively correlated with Gensini score (r = 0.634, P <0.001). After partial correlation analysis of sex, age, hypertension, smoking, body mass index and uric acid, ABI, baPWV and coronary angiography Gensini score There was a correlation of -0.703 and 0.665, respectively (all P <0.001). CONCLUSIONS: ABI and baPWV were increased in patients with coronary heart disease. The ABI and baPWV were negatively correlated with coronary artery disease scores, suggesting that ABI and baPWV could be used as useful indicators to evaluate the severity of coronary artery disease.
