目的探讨接受居家宁养服务与接受住院姑息治疗的晚期癌症患者生活质量的差异及宁养服务的意义。方法选取2016年1-7月新疆某三甲肿瘤医院宁养院和姑息科收治的148例各类晚期癌症患者为研究对象,根据是否接受宁养服务分为居家宁养组(72例)和姑息治疗组(76例),为居家宁养组贫困晚期癌症患者及家属提供居家免费镇痛治疗、心理与社会支持的临终关怀;为姑息治疗组贫困晚期癌症患者提供普通住院模式的对症处理。采用肿瘤患者生活质量评估量表对两组患者接受干预服务后的生活质量进行评价。结果居家宁养组患者接受干预服务前生活质量评分为(36.34±4.59)分,接受干预后为(43.03±1.68)分,差异有统计学意义(P<0.05)。姑息治疗组患者接受干预服务前生活质量评分为(35.21±5.75)分,接受干预后为(40.21±4.33)分,差异有统计学意义(P<0.05)。居家宁养组患者接受干预服务后在家庭的理解与配合、自身对癌症的认识、对治疗的态度3个方面优于姑息治疗组,差异有统计学意义(P<0.05)。结论居家宁养服务为晚期癌症患者提供了一种有效、方便、经济的医疗服务模式,是晚期癌症患者较好的选择。
Objective To explore the differences in the quality of life of patients with advanced cancer accepting home-based nourishment and receiving palliative care and the significance of nourishing services. Methods One hundred and eighty-eight patients with advanced cancer of each type treated in the Ninth Hospital of Xinjiang Cancer Hospital and the Pallidomy Hospital from January to July in 2016 were selected as the study subjects. The treatment group (76 cases) provided home-based free analgesia treatment, psychosocial and social supportive hospice care for the poor patients with advanced stage cancer and their relatives in the Ning-Ning group, and the symptomatic treatment of ordinary hospitalization for poor late cancer patients in the palliative care group. The quality of life of the two groups of patients receiving intervention services was evaluated using the Cancer Patients Quality of Life Scale. Results The scores of quality of life before and after intervention were (36.34 ± 4.59) and (43.03 ± 1.68) points, respectively, with significant difference (P <0.05). The quality of life of patients in palliative group was (35.21 ± 5.75) before intervention and (40.21 ± 4.33) after intervention, the difference was statistically significant (P <0.05). The patients in the home-based nourishment group were better than the palliative group in understanding and cooperating with the family after receiving the intervention service, their awareness of cancer, and their attitude to treatment. There was significant difference (P <0.05). Conclusions Home-ningo-care services provide an effective, convenient and economical mode of medical services for patients with advanced cancer and are a good choice for advanced cancer patients.
