目的:探讨舌癌患者口底淋巴结的发现及转移情况,以引起手术根治中对口底淋巴结的重视。方法:收集华中科技大学附属同济医院口腔颌面外科,在2012年4月至2014年10月期间,73例经病理活检确诊后实施舌颌颈联合根治的舌癌患者。对术中连续切除的组织,进行各区域淋巴结的筛选并分组,通过病理切片检查各个组的转移情况,并采用卡方检验来处理相关数据,判断影响因素的相关性。结果:本研究的73例舌癌患者中,筛查出24例患者存在口底淋巴结,发现口底淋巴结的发现率为32.9%,其中有4例患者口底淋巴结出现肿瘤的转移,转移率为16.7%。所有患者口底及颈部筛选的淋巴结共计863枚,转移的淋巴结共计89枚,主要集中于I~III区及口底区域,并且口底与I-III区的淋巴结转移率相近(P>0.05)。结论:1SCCT患者的口底组织中可能存在转移的口底淋巴结;2口底淋巴结与I-III区的淋巴结转移率相近。
Objective: To investigate the detection and metastasis of lymph node in patients with tongue cancer and to pay more attention to the lymph nodes in the mouth under radical operation. Methods: Tongji Hospital Affiliated to Huazhong University of Science and Technology, Department of Oral and Maxillofacial Surgery, from April 2012 to October 2014 period, 73 patients diagnosed by pathological biopsy of Tongue and Neck joint radical mastectomy tongue cancer patients. The lymph nodes were screened and grouped in each region of the continuous resection. The metastasis of each group was examined by pathological sections. The chi-square test was used to deal with the related data to determine the correlation of the influencing factors. Results: Among the 73 cases of tongue cancer patients, 24 cases were found with lymph nodes in the mouth, and 32.9% were found in the mouth lymph nodes. Among them, 4 cases had lymph node metastasis, the rate of metastasis was 16.7%. All patients had 863 lymph nodes screened at the mouth and neck, and 89 lymph node metastases, which mainly concentrated in the area of I ~ III and the floor of the mouth, and the lymph node metastasis rate was similar between the floor and the I-III area (P> 0.05 ). Conclusions: Mouth lymph node metastasis may exist in the mouth and floor of patients with 1SCCT. The rate of lymph node metastasis in 2-node lymph node is similar to that of I-III.
