目的探讨2型糖尿病患者血浆同型半胱氨酸(tHcy)与相关临床及生化指标的相关性,为预防2型糖尿病并发症提供依据。方法选择2015年1-12月住院的206例2型糖尿病患者作为研究对象,根据tHcy水平将206例2型糖尿病患者分为tHcy>15μmol/L组(51例,高同型半胱氨酸组)和tHcy≤15μmol/L组(155例,正常同型半胱氨酸组)。选择来自健康体检中心的40例健康体检者作为健康对照组。测定研究对象tHcy、血糖、血脂、肌酐、尿酸、糖化血清蛋白、糖化血红蛋白、总蛋白和白蛋白等指标。用SPSS 19.0软件对数据进行t检验和Pearson相关分析。结果 2型糖尿病组tHcy水平[(16.4±6.3)μmol/L]明显高于健康对照组[(7.8±2.3)μmol/L],差异有统计学意义(t=-10.8,P<0.01)。与正常血浆同型半胱氨酸组相比,高同型半胱氨酸组空腹血糖、肌酐、tHcy、尿酸、前白蛋白和糖化血红蛋白差异均有统计学意义(P<0.05,P<0.01)。Pearson相关分析结果显示,tHcy与低密度脂蛋白胆固醇、肌酐、尿酸和糖化血红蛋白均呈明显的正相关(r值分别为0.142、0.450、0.378和0.233,P<0.05,P<0.01)。结论低密度脂蛋白胆固醇、肌酐、尿酸和糖化血红蛋白是2型糖尿病患者tHcy的重要相关因素,检测这些指标能更早地预测糖尿病肾病等并发症。
Objective To investigate the correlation between plasma homocysteine (tHcy) and related clinical and biochemical parameters in type 2 diabetes mellitus (T2DM) and provide basis for prevention of type 2 diabetes mellitus. Methods Totally 206 patients with type 2 diabetes mellitus admitted from January to December in 2015 were enrolled. According to the level of tHcy, 206 patients with type 2 diabetes were divided into tHcy> 15μmol / L group (51 cases, high homocysteine group) And tHcy≤15μmol / L group (155 cases, normal homocysteine group). Forty healthy subjects from the Health Check Center were selected as healthy control group. Subjects were measured tHcy, blood glucose, blood lipids, creatinine, uric acid, glycosylated serum protein, glycosylated hemoglobin, total protein and albumin and other indicators. Data were analyzed by t-test and Pearson correlation using SPSS 19.0 software. Results The level of tHcy in type 2 diabetic patients was significantly higher than that in healthy controls [(16.4 ± 6.3) μmol / L vs (7.8 ± 2.3) μmol / L, t = -10.8, P <0.01). Compared with normal plasma homocysteine group, the differences of fasting blood glucose, creatinine, tHcy, uric acid, prealbumin and glycosylated hemoglobin in high homocysteine group were statistically significant (P <0.05, P <0.01). Pearson correlation analysis showed that tHcy was positively correlated with LDL cholesterol, creatinine, uric acid and HbA1c (r = 0.142,0.450,0.378and0.233, respectively, P <0.05, P <0.01). Conclusion Low density lipoprotein cholesterol, creatinine, uric acid and glycosylated hemoglobin are important related factors of tHcy in type 2 diabetic patients. Detecting these indexes can predict complications such as diabetic nephropathy earlier.
