为了解角膜缘切口的白内障超声乳化吸出及后房折叠人工晶体植入术 ( PEA+FIOL术 )的角膜屈折力的变化 ,采用上方角膜缘 3 .2 mm隧道切口 ,不缝合 ,对 3 5例 3 9眼白内障行 PEA+FIOL术 ,用角膜曲率计测量术前及术后不同时期的角膜屈折力。结果术后第一天出现逆规性散光者占 66.67% ,1~ 3个月仍占 5 0 % ;术后一个月两主径线角膜屈折力差平均为 1.78D± 1.2 2 ,比术前增加 0 .5 9D ( P<0 .0 1) ;术后 3个月平均为 1.10 D± 0 .86,比术前减小 0 .0 9D ( P<0 .0 1)。结论 :经上方角膜缘切口的无缝合PEA+FIOL术 ,可产生一定量的逆规性散光 ,随时间的延长而逐渐减小 ,术后 3个月恢复到术前水平甚至小于术前散光。
In order to understand the changes of corneal refractive power in corneal incision cataract phacoemulsification and posterior chamber intraocular lens implantation (PEA + FIOL), the 3.2 cm tunnel incision of the upper limbal margin was used, 39 eyes of cataract patients underwent PEA + FIOL surgery. Corneal refractive power was measured by keratometer before and after surgery. Results The first day after operation, 66.67% had astigmatism, and still accounted for 50% after 1 ~ 3 months. The mean corneal refractive power difference between the two major diameters was 1.78D ± 1.2 2 in one month after operation, (P <0.01). The average postoperative 3 months was 1.10 D ± 0.86, which was 0.90D lower than that before operation (P <0.01). Conclusion: The seamless corneal edge incision PEA + FIOL can produce a certain amount of contracted astigmatism, which decreases gradually with the extension of time. The postoperative recovery to preoperative level is even less than preoperative astigmatism.
