肝脏的切除一般均需剥离肝门部肝蒂的血管并结扎,有时很困难,手术并发症和死亡率也较高。作者提出一种左半肝切除的简单方法,在肝门部将左半肝的血管胆管蒂整个结扎,而不分别剥离血管和胆管。解剖基础:左半肝的血管胆管蒂(肝蒂)较长,因左右分义处位于肝门的右侧,故左肝蒂全部经过肝门,在方叶之后,尾叶之前,至肝门和左纵沟交介处,然后弯向前方,在脐静脉沟内走行形成左正中旁蒂系。在弯向前方处分出一左外侧支,供应左肝的后部,另有数小支由后侧分出至尾叶,上方则无分支。
