目的:了解吉林市乡村护理人员道德困境、职业倦怠的现状。方法:在吉林市周围乡村随机选取4家医院的389名护理人员进行道德困境和职业倦怠的问卷调查。结果:乡村护理人员道德困境总分为(45.67±13.05)分,各维度得分:个体责任(10.79±3.43)分、未能维护患者最大利益(12.64±4.35)分、价值冲突(19.12±5.47)分、损害患者利益(4.42±1.26)分,道德困境强度(0.94±0.27)分,道德困境频率(1.04±0.37)分,道德困境处于轻度水平。护理人员的职业倦怠总分为(60.93±18.22)分,各维度得分:情绪耗竭(25.73±12.13)分,去人格化倾向(7.79±6.61)分和个人成就感(26.39±10.83)分,职业倦怠处于中轻度水平。结论:吉林市周围乡村医院护理人员总体道德困境与职业倦怠呈低、中低水平,30岁以上年龄组道德困境与职业倦怠水平明显高于30岁以下年龄组,35岁以上年龄组道德困境与情感耗竭最为严重,说明随着年龄的增长道德困境与职业倦怠水平明显增加,相关管理部门应引起重视,采取切实可行的方案,解决乡村护理人员道德困境和职业倦怠的相关问题。
Objective: To understand the status quo of moral dilemma and job burnout among rural nurses in Jilin City. Methods: A total of 389 nurses from 4 hospitals were randomly selected from the villages around Jilin City to conduct a questionnaire survey of moral dilemmas and job burnout. Results: The total scores of ethical dilemmas in rural nurses were (45.67 ± 13.05) points, the scores of individual dimensions were: individual responsibility (10.79 ± 3.43), failing to maintain the maximum benefit of patients (12.64 ± 4.35), value conflict (19.12 ± 5.47) (4.42 ± 1.26), moral dilemma (0.94 ± 0.27), moral dilemma (1.04 ± 0.37) points, moral dilemma at a mild level. The total score of job burnout of nursing staff was (60.93 ± 18.22) points, the scores of each dimension were: emotional exhaustion (25.73 ± 12.13), personality depersonalization (7.79 ± 6.61) and personal accomplishment (26.39 ± 10.83), job burnout At a mild level. Conclusion: The ethical dilemmas and job burnout of nursing staff in rural hospitals around Jilin City are low and medium level. The moral dilemma and job burnout in those over 30 years old are obviously higher than those under 30 years old. Emotional exhaustion is the most serious, indicating that with age increasing moral dilemma and job burnout levels significantly increased, the relevant management departments should pay attention to take practical measures to solve the moral problems of rural nurses and job burnout related issues.
