目的观察丹参多酚酸盐对老年冠心病心绞痛的疗效及对患者血清血管内皮生长因子(VEGF)、基质金属蛋白酶-9(MMP-9)水平的影响。方法选取80例老年冠心病心绞痛患者,随机分为对照组和治疗组各40例。2组均给予常规治疗,治疗组在常规治疗基础上静脉滴注丹参多酚酸盐,疗程14d。观察2组临床疗效、心电图改善情况、西雅图心绞痛量表评分(SAQ)及不良反应。采用荧光定量PCR(Real time-PCR)检测2组治疗前后血清VEGF、MMP-9水平,并与35例健康人作为正常组进行比较。结果治疗组总有效率、心电图改善情况、SAQ评分均优于对照组,差异有统计学意义(P<0.05),2组均无不良反应发生;治疗前2组血清VEGF、MMP-9水平高于正常组,经治疗后2者水平均明显降低,与治疗前比较差异有统计学意义(P<0.05);且治疗组治疗后VEGF、MMP-9水平降低更为显著,与对照组治疗后比较差异有统计学意义(P<0.05)。结论丹参多酚酸盐治疗老年冠心病心绞痛安全有效,同时降低患者血清VEGF、MMP-9水平,这可能是丹参多酚酸盐发挥治疗作用的机制之一。
Objective To observe the curative effect of salvianolate on angina pectoris in elderly patients with coronary heart disease and its effect on serum levels of vascular endothelial growth factor (VEGF) and matrix metalloproteinase-9 (MMP-9). Methods 80 elderly patients with coronary heart disease angina pectoris were randomly divided into control group and treatment group of 40 cases. Both groups were given routine treatment. The treatment group received intravenous salvianolate injection on the basis of routine treatment for 14 days. The clinical efficacy, electrocardiogram improvement, Seattle angina pectoris scale (SAQ) and adverse reactions were observed in two groups. The levels of VEGF and MMP-9 in two groups before and after treatment were detected by Real-time PCR, and compared with 35 normal subjects. Results The total effective rate, the improvement of electrocardiogram and the SAQ score in the treatment group were better than those in the control group (P <0.05). No adverse reactions occurred in the two groups. The levels of VEGF and MMP-9 in the two groups before treatment were high The levels of VEGF and MMP-9 in the treatment group were significantly lower than those in the control group after treatment (P <0.05), and the levels of VEGF and MMP-9 in the treatment group were significantly lower than those in the control group The difference was statistically significant (P <0.05). Conclusion Salviae miltiorrhizae polyphenols is safe and effective in the treatment of angina pectoris in elderly patients, and at the same time reduce the serum levels of VEGF and MMP-9, which may be one of the mechanisms of Salvia miltiorrhiza.
