胼胝体后部海绵状血管瘤卒中,迄今文献报道很少。我院神经外科自1987～1989年底,在53例幕上占位病变手术中,遇有2例为胼胝体后部海绵状血管瘤卒中,手术切除后,均得以康复出院,现报告如下。例1.女,42岁。于1989年9月11日入院。患者入院前3小时,吃饭时突然感到头痛欲裂,持续1小时后,突然意识不清,喷射状频繁呕吐,小便失禁,呼吸困难。急诊以高血压脑出血合并上消化道出血收住神经内科。CT扫描后疑为幕上脑瘤卒中转神经外科。既往有高血压病史10余年。检查:血压28/24kPa。昏迷,呼吸困难。双侧瞳孔直径4mm,圆光反应迟钝。双侧眼底视神经乳头轻度水肿。左侧鼻唇沟变浅。左侧肢体肌张力增高,腱反射(亢进),Babinski氏征阳性。CT增强扫描示:右顶枕叶区脑内血管破裂出血,血肿形成。腰穿为血性脑脊液。1989年9月16日全麻开颅探查。术中颅内压极高,右顶上小叶后部,脑表面有一约2cm×2.5cm大暗紫色出血病灶,局部向外突出,穿刺阴性。在暗紫色区中间电凝后切开脑组织。在距脑表面1cm深处发现血肿,清除积血

Bilateral fundus optic nerve head mild edema. The left nasolabial fissure shallow. Left limb muscle tone increased, tendon reflex (hyperthyroidism), Babinski’s sign positive. CT enhanced scan showed: the right top of the occipital lobe brain rupture of blood vessels, hematoma formation. Waist wear bloody cerebrospinal fluid. September 16, 1989 general anesthesia craniotomy. High intracranial pressure during surgery, the top of the right top of the leaflets, the brain surface has a large dark purple bleeding 2cm × 2.5cm lesions, local outwards, puncture-negative.
