目的探讨血I型胶原氨基端前肽(PINP)和I型胶原羧基端肽β特殊序列(β-CTX)在老年高血压合并慢性阻塞性肺疾病急性期(AECOPD)中的表达。方法选择2011年1月至2013年12月124例老年高血压合并慢性阻塞性肺病(COPD)患者为研究对象。将124例患者按照COPD严重程度分为AECOPD期64例(急性加重组),COPD缓解期60例(缓解组),同期选择72例健康人群作为对照组。给予AECOPD患者促进骨形成药物治疗,检测受试者血清PINP、β-CTX含量,并对老年高血压合并AECOPD患者PINP、β-CTX的表达与年龄、烟龄、肺功能、体重指数(BMI)的关系进行相关性分析。结果急性加重组血清PINP、β-CTX含量明显低于缓解组和对照组,差异有统计学意义(P<0.01),缓解组PINP水平低于对照组,但β-CTX高于对照组,差异有统计学意义(P<0.01)。AECOPD患者经治疗后血清PINP、β-CTX水平明显较治疗前提高,差异有统计学意义(P<0.01)。老年高血压合并AECOPD血PINP表达与烟龄呈负相关(P<0.05),与FEV1%预计值、FEV1/FVC(%)呈正相关(P<0.05);血β-CTX表达与烟龄呈负相关(P<0.05),与FEV1%预计值、FEV1/FVC(%)呈正相关(P<0.05);血PINP和β-CTX表达之间呈正相关(P<0.05)。结论老年高血压合并AECOPD患者血PINP、β-CTX表达明显降低,患者骨质疏松症发病风险增大。
Objective To investigate the expression of PINP and β-CTX in type A collagen in patients with hypertension and chronic obstructive pulmonary disease (AECOPD). Methods A total of 124 elderly patients with chronic obstructive pulmonary disease (COPD) from January 2011 to December 2013 were selected as study subjects. According to the severity of COPD, 124 patients were divided into AECOPD 64 cases (acute exacerbation group), COPD remission 60 cases (remission group), the same period 72 healthy people as control group. The patients were given AECOPD to promote bone formation drug therapy. The contents of PINP and β-CTX in serum of patients with AECOPD and the age, smoking age, lung function, body mass index (BMI) The relationship between the correlation analysis. Results The levels of PINP and β-CTX in acute exacerbation group were significantly lower than those in remission group and control group (P <0.01). The level of PINP in remission group was lower than that in control group, but β-CTX was higher than that in control group There was statistical significance (P <0.01). The serum levels of PINP and β-CTX in AECOPD patients after treatment were significantly higher than those before treatment (P <0.01). There was a negative correlation between blood PINP and smoking age (P <0.05), and the FEV1 / FVC (%) was positively correlated with FEV1% predicted value (P <0.05). The blood β-CTX expression was negative (P <0.05). There was a positive correlation between FEV1 / FVC (%) and predicted FEV1% (P <0.05). There was a positive correlation between blood PINP and β-CTX expression (P <0.05). Conclusions The expression of PINP and β-CTX in elderly patients with AECOPD is significantly lower and the risk of osteoporosis is increased.
