目的探讨环氧合酶-2(COX-2)在脓毒症大鼠肝组织炎症反应中的表达及其对代谢反应的影响。方法选择体质量相近的Wistar大鼠54只随机分为3组。A组为假手术组;B组为脓毒症模型组;C组为B组+NS-398干预组。通过建立盲肠结扎穿孔(CLP)脓毒症动物模型,用RT-PCR方法检测各组肝组织COX-2mRNA的表达;采用放射免疫法检测前清蛋白(PA)、转铁蛋白(Tf);同时检测ALT,AST和血浆清蛋白(Alb),并观察肝脏病理变化。结果(1)COX-2mRNA在A组呈低表达,B,C组表达明显增高,但C组较B组各时点明显降低(P<0.05);(2)ALT和AST在B组各时点改变较A,C组明显增高(P<0.05);(3)B组病理损伤重,C组肝脏病理损伤减轻;(4)B,C组PA,Tf,Ab较A组下降明显,但C组各时点显著高于B组(P<0.05)。结论COX-2在脓毒症肝损伤中发挥重要作用,它促进分解代谢,抑制合成代谢;其选择性抑制剂可以减轻肝损伤,减弱蛋白分解。
Objective To investigate the expression of cyclooxygenase-2 (COX-2) in inflammatory reaction of liver tissue and its effect on metabolism in septic rats. Methods Fifty-four Wistar rats with similar body weight were randomly divided into three groups. Group A was sham operation group; Group B was sepsis model group; Group C was Group B + NS-398 intervention group. The animal model of cecal ligation and puncture (CLP) sepsis was established. The expression of COX-2 mRNA in each group was detected by RT-PCR. Pre-PA and Tf were detected by radioimmunoassay ALT, AST and serum albumin (Alb) were detected, and liver pathological changes were observed. Results (1) The expression of COX-2 mRNA in group A was significantly lower than that in group B (P <0.05). The expression of COX-2 mRNA in group B and C was significantly higher than that in group B (4) The PA, Tf, Ab in group B and C were significantly lower than that in group A, but the level of PA, Tf and Ab in group B and C were significantly lower than that of group A (P <0.05) The time points in group C were significantly higher than those in group B (P <0.05). Conclusions COX-2 plays an important role in hepatic injury induced by sepsis. It promotes catabolism and inhibits anabolism. Its selective inhibitors can reduce liver injury and attenuate proteolysis.
