目的:分析我院门诊中药房处方调剂差错发生的原因及相关预防措施。方法:选择2012-2014年笔者所在门诊中药房处方作为研究对象,从2012-2014年中随机抽取一个月的处方进行分析,对存在差错的处方进行统计和原因分析并制定预防措施。结果:2012年至2014随机抽取每年3月份的处方,共存在1584例处方调剂差错事件。2012年3月存在561例,2013年3月存在526例,2014年3月存在497例。所有处方调剂差错中,品种差错516例,占所有差错32.6%;数量差错372例,占所有差错23.5%;错发患者差错312例,占所有差错19.7%;用法用量差错228例,占所有差错14.4%;其他差错156例,占所有差错9.8%。结论:分析引起处方调剂差错的原因,并采取有效预防措施,规范门诊中药房管理,降低调剂差错发生率。
OBJECTIVE: To analyze the causes of the errors in prescription prescriptions and the related preventive measures in our outpatient clinic. Methods: From 2012 to 2014, we selected prescription pharmacies in the outpatient clinic from 2012 to 2014 as the research object. We randomly selected a one-month prescription during 2012-2014 for analysis, analyzed the causes and analyzed the causes of the errors, and formulated preventive measures. Results: From March 2012 to March 2014, prescriptions were randomly selected from March to March. A total of 1584 prescription errors occurred. There were 561 cases in March 2012, 526 cases in March 2013 and 497 cases in March 2014. Among all the prescription adjustment errors, 516 varieties accounted for 32.6% of all errors; 372 cases accounted for 23.5% of all errors; 312 cases of mistakes were caused by mistakes, accounting for 19.7% of all errors; 228 cases of errors in usage accounted for all errors 14.4%; 156 other errors, accounting for 9.8% of all errors. Conclusion: This paper analyzes the causes of errors caused by prescriptions, and takes effective preventive measures to standardize pharmacy management in outpatients and reduce the incidence of dispensing errors.
