一、尿的生成过程和利尿药作用部位尿的生成过程可以分为三个环节: 1.肾小球滤过血液流经肾小球肘,除蛋白质及血细胞外,其他成分都被滤过形成原尿(表1)。正常人肾小球滤过率为125ml/分,每日原尿达180升。滤过压为60mmHg,滤过压=肾小球毛细血管压-血浆胶体渗透压-肾小球囊内压。正常情况下肾小球滤过率保持相对稳定。一般情况下肾小球滤过和近曲小管重吸收是成比例改变的,因此仅增加肾小球滤过的药物并无明显的利尿作用。但是当滤过率明显降低时,多数利尿药效果又较差。
