目的探讨颅骨钻孔引流治疗高血压脑出血的方法及疗效。方法对285例高血压脑出血患者行头皮切开、颅骨钻孔、直视下止血,置特制血肿腔冲洗引流管,抽吸、引流血肿,加尿激酶应用。结果根据ADL分级法,本组病例恢复至Ⅰ级63例(占22.1%),Ⅱ级96例(占33.7%),Ⅲ级48例(占16.8%),Ⅳ级33例(占11.6%),Ⅴ级27例(占9.4%),死亡18例(占6.3%)。结论应用颅骨钻孔,置特制血肿腔冲洗引流管,抽吸加引流及血肿腔内注入尿激酶,治疗高血压脑出血,能较彻底清除血肿,迅速缓解颅内压,取得较好的治疗效果。
Objective To investigate the method and effect of craniotomy drainage for hypertensive intracerebral hemorrhage. Methods 285 patients with hypertensive intracerebral hemorrhage underwent scalp incision, skull drilling and hemostasis under direct vision. A special hematoma drainage tube was drained, aspiration and drainage hematoma were added, and urokinase was applied. Results According to ADL classification, 63 cases (22.1%) recovered to grade Ⅰ, 96 cases (33.7%) in grade Ⅱ, 48 cases (16.8%) in grade Ⅲ and 33 cases (11.6%) in grade Ⅳ , Grade V 27 cases (9.4%), 18 cases died (6.3%). Conclusion The application of skull drilling, special hematoma drainage drainage tube, suction and drainage and hematoma cavity injection of urokinase, treatment of hypertensive intracerebral hemorrhage, more thorough removal of hematoma, rapid intracranial pressure relief and achieve better therapeutic effect .
