目的通过分析1名铟冶炼操作工矽肺的临床表现及X射线、病理、肺功能检查等变化,探讨铟致矽肺患者的疾病转归。方法回顾分析1名铟冶炼操作工矽肺发生、发展8年的临床资料。结果该铟冶炼操作工接触铟尘3年5个月后被诊断为贰期矽肺,发病初期无明显临床症状,肺功能及其他实验室检查未发现异常;从贰期矽肺晋期到叁期矽肺约为4年时间,并有反复咳嗽、咳痰、活动后胸闷、气紧等临床症状。支气管慢性炎症改变,肺组织可见少量尘灶、纤维组织轻度增生和胶原纤维形成,肺通气功能大致正常、弥散功能轻度降低。结论该铟冶炼工矽肺晋期年限短,应加强铟作业工人的长期随访,以便进一步观察铟肺病的自然史,有效预防铟所致矽肺的发生。
Objective To analyze the clinical manifestation of silicosis of an Indium smelting operator and the changes of X-ray, pathology and pulmonary function tests to explore the prognosis of patients with silicosis caused by indium. Methods Retrospective analysis of an Indium smelting operator silicosis, the development of 8 years of clinical data. Results The indium smelting operator contacted with indium dust for 3 years and 5 months after being diagnosed as stage II silicosis. There was no obvious clinical symptom in the early stage of onset, and no abnormalities were found in pulmonary function and other laboratory tests. From the stage of advanced silicosis of stage II to the stage of silicosis About 4 years, and there are repeated cough, sputum, chest tightness after activities, gas tight and other clinical symptoms. Chronic bronchial inflammation changes, a small number of dust visible in the lung tissue, fibrous tissue mild hyperplasia and collagen fibers, pulmonary ventilation function is generally normal, slightly reduced dispersion. Conclusion The indium smelting workers should pay more attention to the long-term follow-up of indium workers in order to further observe the natural history of indium-induced lung disease and effectively prevent the occurrence of silicosis caused by indium.
