目的探讨自制改良光导气管导管在人体模型体上进行气管插管训练的性能及应用前景。方法在普通气管插管导管的结构基础上,选用软硬适中的优质塑胶材料进行制作,将光导纤维植入导管体管壁,光导纤维输入端和输出端分别连接分光器和聚光器,导管外接口端固化自带光源发出光线,经分光器、光导纤维、聚光器照亮导管前端区域,使插管过程气道照明充足。按临床常用气管导管的规格、消毒标准制作出光导导管。抽取该院急诊科医师8人在人体模型身上进行气管插管训练,每人插管100次,记录人均一次插管成功率、人均每次插管时间,并与对照组比较。结果观察组一次插管成功87~96例,人均93.5例;成功率为87%~96%,人均93.65%;人均每次插管时间(11.59±0.38)s。对照组一次插管成功83~95例,人均89例;成功率为83%~95%,人均90.03%;人均每次插管时间(13.14±0.38)s。两组同性指标分别比较,差异均有统计学意义(P<0.01)。结论自制改良光导气管导管操作便捷、一次插管成功率高、耗时短,值得临床探讨。
Objective To explore the performance and application of self-made modified light-guiding tracheal tube in tracheal intubation training on human body model. Methods Based on the structure of general tracheal intubation catheter, we made the best plastic material with moderate hardness. The optical fiber was implanted into the tube wall of the catheter. The input and output ends of the optical fiber were connected to the optical splitter and the condenser respectively. The external interface-end curing comes with a light source to emit light, which is illuminated by a beam splitter, optical fiber and a condenser to illuminate the airway of the intubation process. According to the commonly used clinical tracheal catheter specifications, disinfection standards to produce a light guide catheter. The hospital emergency department physician extracted eight tracheal intubation training in the body model, each intubation 100 times, record the success rate of per capita intubation, per capita per intubation time, and compared with the control group. Results The success rate of primary intubation was 87-96 cases (93.5%) in the observation group. The success rate was 87% -96% with a mean of 93.65%. The average per-intubation time was 11.59 ± 0.38 s. In the control group, 83 to 95 cases of primary intubation were successfully performed, with a per capita average of 89 cases. The success rate was 83% -95% with a mean of 90.03%. The average per-intubation time was 13.14 ± 0.38 s per capita. The two groups of same-sex indicators were compared, the differences were statistically significant (P <0.01). Conclusion Self-made modified light-guided tracheal catheter is easy to operate and has a high successful rate of intubation and short time-consuming. It is worth to be discussed clinically.
