透明质酸刺激因子对深II度烧伤创面瘢痕形成的影响

从兔羊水及胎兔血清中提取透明质酸刺激因子(HASF)液。将其用于深Ⅱ度烧伤创面,采用自身对照法,通过观测早期创面渗液中透明质酸(HA)含量变化、愈合后一个月局部组织的外观及镜下观和组织中胶原含量的分析,从中探讨了HASF在深Ⅱ度烧伤创面愈合过程中对瘢痕形成的影响。此外还将HASF用于体外培养的兔成纤维细胞,观测上清液中HA含量的改变。结果表明,HASF可以明显提高创面渗液中HA的含量,体外实验也证实HASF有促成纤维细胞产生HA的作用,而HA具有调节胶原合成的作用,可提高Ⅲ型胶原的比率,使胶原纤维变细,从而减轻瘢痕的产生。此外发现,在瘢痕组织中,Ⅰ、Ⅲ型胶原总量和正常皮肤间差异无显著意义(P>0.05)。 Hyaluronic acid stimulating factor (HASF) fluid was extracted from rabbit amniotic fluid and fetal rabbit serum. This method was applied to the deep second degree burn wounds. By using the self-control method, the change of the content of hyaluronic acid (HA) in early wound exudate, the appearance of the local tissue one month after healing, and the collagen content in the microstructure were observed The effect of HASF on the formation of scar in deep second degree burn wound healing was discussed. In addition, HASF was also used to culture rabbit fibroblasts in vitro, and the changes of HA content in the supernatant were observed. The results show that, HASF can significantly improve the wound exudate HA content, in vitro experiments also confirmed that HASF fibroblasts produce HA, and HA has the role of regulation of collagen synthesis, can increase the type Ⅲ collagen ratio, so that the collagen fibers change Fine, thus reducing the generation of scars. In addition, there was no significant difference between the total amount of type Ⅰ and type Ⅲ collagen and normal skin in scar tissue (P> 0.05).

检索了1983年1月~1992年12月收治的3911例烧伤病人的病历资料,对一般情况、年龄分布、致伤原因、烧伤面积、复合伤、常见并发症、休克发生率、死亡率及死亡原因等进行了统计分析。结果显示,在132例死亡病人中,91.7%有吸入伤、内脏并发症和全身性感染。提示吸入伤、内脏并发症和全身性感染仍是烧伤病人的三大主要死亡原因。
用白色雄性家兔45只,制成15%Ⅲ度烫伤后绿脓杆菌(PA)感染模型,分为预防、治疗和对照组三组。伤后立即静脉注射头孢哌酮(CPZ)组(预防组),在伤后1周内创面结痂、干燥,免疫荧光抗体染色见PA仅位于焦痂浅层;而第48小时开始静脉注射CPZ组(治疗组),则在伤后第3、4天PA即侵入焦痂深层肌组织。血液细菌培养阳性率和动物病死率后者明显高于前者,且与对照组结果相同。这提示严重烧(烫)伤后必须在渗出早
